Özet

Yüz travması sonrası sinüs lümeninde kan birikmesi, çoğunlukla komşu duvar veya orbita tabanı kırığına eşlik eder. Kontrastsız yüz kemikleri BT'sinde sinüs lümenindeki kan, yüksek dansiteli sıvı veya hava-kan seviyesi olarak izlenir ve komşu kemik kırığı açısından uyarıcı bir bulgudur. Sinüs opasifikasyonu kırık için uyarıcıdır ancak tek başına kırık yerini göstermez ve enfeksiyonla da karışabilir. Hematosinüsü fark edip çevre kemikleri taramamak, eşlik eden orbital veya orta yüz kırığını atlatabilir.

Kontrastsız tanısal

Akut kan sinüs havasına göre hiperdens içerik oluşturur; kısmi opasifikasyon veya hava-kan seviyesi görülebilir. Kontrast gerekmez; kemik algoritmalı ince kesit ve çok düzlemli rekonstrüksiyon tanısaldır.

BT bulguları

• Sinüs içinde çevre havadan belirgin yüksek dansiteli sıvı veya pıhtı.
• Maksiller sinüste yatay hava-sıvı seviyesi ya da tabakalanan kan görünümü.
• Sinüs opasifikasyonu, sinüs duvarı kırığıyla ilişkili olabilir; kırık hattı BT'de ayrıca aranmalıdır.
• Orbita tabanında kırıkla birlikte maksiller sinüse uzanan kan.
• Etmoid sinüs hücresinde opasifikasyon veya hava-sıvı seviyesi gözlenir.
• Sinüs içeriğinin heterojen dansitesi enfeksiyöz etiyolojiyi düşündürebilir; sinüs opasifikasyonu veya hava-sıvı seviyesi tek başına travmatik kanamayı kanıtlamaz.

Normalde

Sinüs BT'sinde çevresel mukozal kalınlaşma sıvı opasifikasyonu sayılmaz; retansiyon kistleri ayrıca görülebilen bir maksiller sinüs bulgusudur. Travmada yeni sinüs içi sıvı, kırık aramayı gerektiren dolaylı bir ipucudur.

Tuzaklar

• Sinüs opasifikasyonunu doğrudan kırık diye adlandırma; kemik penceresinde her duvarı ayrı izleyip kırık hattını göster.
• Kronik rinosinüzit bulguları varsa, maksiller antrumdaki kalsifikasyonları ve sinüs duvarlarındaki osteiti dikkate alarak sinüs içeriğini yorumlayın.
• İzole nazal kemik yaralanması sinüs kanaması yapmayabilir; hematosinüs saptandığında orta yüz ve orbita kemiklerini de kapsa.
