Özet

Dış kulak yolu osteomu çoğunlukla yavaş büyüyen, tek taraflı ve tesadüfen saptanan benign bir kemik çıkıntısıdır; kanal lümenini daraltırsa iletim tipi işitme yakınmalarına veya sekonder keratin birikimine yol açabilir. İnce kesit kontrastsız temporal kemik BT'si odağın kemik kökenini, kanal duvarıyla bağlantısını ve lümen üzerindeki etkisini gösterir. Tek, saplı ve genellikle lateral yerleşimli görünüm, bilateral çoklu ekzostozlardan ayrımda yardımcıdır. Kanalın ileri daralması ya da birlikte bulunan kolesteatom fark edilmezse lezyonun işlevsel etkisi ve kemik erozyonu gözden kaçabilir.

Faz ve pencere

Kontrastsız tanısal
İnce kesit görüntülerde dış kulak yolu duvarından lümene uzanan kortikal kemik atenüasyonunda, keskin sınırlı fokal çıkıntı izlenir; lezyonun lümeni kapatıp kapatmadığı ve orta kulağa uzanıp uzanmadığı değerlendirilir. Kontrast fazı tanı için gerekli değildir.
Kemik algoritması tanısal
Aksiyel ve ortogonal rekonstrüksiyonlar saplı tutunmayı, timpanoskuamöz veya timpanomastoid sütür komşuluğunu ve kanal korteksinin devamlılığını belirginleştirir.

Önerilen pencereler: Kemik (G 1800 / M 400).

BT bulguları

  • Tek taraflı yoğun kemik çıkıntısı — dış kulak yolu lümenine doğru belirgin biçimde uzanır.
  • Saplı tutunma — kemik odağı kanal duvarına dar bir tabanla bağlanır.
  • Lateral kanal yerleşimi — tipik olarak istmusun lateralinde ve timpanoskuamöz ya da timpanomastoid sütür yakınındadır.
  • Korunmuş kanal korteksi — lezyon çevresinde agresif destrüksiyon veya düzensiz permeatif erozyon beklenmez.
  • Lümen daralması — karşı kulak kanal çapı ve lezyonun timpanik zara uzaklığı ile birlikte kaydedilir.
  • İkincil kolesteatom şüphesi — dar kanalın medialinde keratin birikimi ve eşlik eden fokal kemik erozyonu aranır.

Normalde

Normal dış kulak yolunda kıkırdak bölüm lateralde, kemik bölüm medialde düzgün kortikal duvarlarla çevrilmiş açık bir lümendir ve timpanik membrana kesintisiz uzanır. Aynı kesitte karşı tarafı kıyaslayarak kanal lümenine taşan tek bir saplı kemik odağı olup olmadığına, tabanının genişliğine ve duvarda erozyon bulunup bulunmadığına bakın.

genellikle bilateral, çoklu, simetrik ve geniş tabanlı kemik kabarıklıkları yapar.
Dış kulak yolu kolesteatomu
kemik yoğunluğunda çıkıntıdan çok yumuşak doku/keratin kitlesi ve komşu kanal duvarında erozyon oluşturur.
Serumen tıkacı
osteomun medialinde birikebilir ve kanal tıkanıklığına katkıda bulunabilir.

Tuzaklar

  • Geniş tabanlı ya da birden çok kemik çıkıntısını otomatik olarak osteom saymayın; bilateral simetri ve medial kanal yerleşimi ekzostozu destekler.
  • Kanal lümenindeki yumuşak doku odağını kemik çıkıntısına bağlamayın; aksiyel ve koronal kemik rekonstrüksiyonlarında kortikal devamlılığı izleyin.
  • Daralmış kanalın medial ucunda biriken keratini gözden kaçırmayın; osteoma eşlik eden kolesteatom duvar erozyonuna neden olabilir.
