Temporal kemik Paget hastalığı BT bulguları

Özet

Paget hastalığı çoğunlukla ileri yaşta, kemik dönüşümünün arttığı kronik bir hastalıktır; temporal kemik tutulumu işitme kaybı veya tesadüfi görüntüleme bulgusu ile fark edilebilir. Yüksek çözünürlüklü BT, petroz kemikte genişleme, kortikal kalınlaşma ve litik-sklerotik yeniden yapılanmayı gösterir; otik kapsül ve iç kulak yapılarının etkilenme derecesini de ortaya koyar. BT bulguları sistemik Paget hastalığını tek başına kanıtlamaz; dağılım ve klinik/laboratuvar bağlamı ile yorumlanır. Hastalığın otik kapsüle uzanımı ya da eşlik eden kemik deformasyonu atlanırsa işitme yakınmasının yapısal nedeni ve cerrahi anatomisi eksik değerlendirilir.

Faz ve pencere

Kontrastsız tanısal: İnce kesit kemik algoritmasında petroz temporal kemikte genişleme, kortikal kalınlaşma ve yamalı litik-sklerotik trabeküler yeniden yapılanma aranır; bu kemik hastalığının değerlendirilmesinde kontrast gerekli değildir.

Önerilen pencereler: Kemik (G 1800 / M 400).

BT bulguları

Petroz kemik ve mastoid bölümde asimetrik hacim artışı, komşu kafa tabanı konturunu genişletebilir.
Kortikal tabaka kalınlaşır; iç ve dış korteksin sınırları kaba yeniden yapılanma nedeniyle düzensizleşebilir.
Litik alanlarla yoğun sklerotik odakların birlikte bulunması, yamalı ‘pamuk yünü’ görünümü oluşturabilir.
Trabeküller kalın ve kaba görünür; kemik iliği boşlukları olağan ince trabeküler deseni kaybeder.
Otik kapsül çevresindeki dansite heterojenliği ve demineralizasyon, iç kulak çevresinde hastalık yayılımını düşündürür.
Kemik yeniden yapılanması krall sinir yollarını sıkıştırarak nörolojik defisitlere neden olabilir; kanal genişlemesi veya daralması değerlendirilir.
Temporal kemik BT'sinde orta kulak ve mastoid yapılar ayrıca sistematik olarak gözden geçirilmelidir.

Ölçütler ve sınıflamalar

Klasik kemik aktivite fazları: Osteolitik fazda rezorpsiyon, mikst fazda rezorpsiyon ve yeni kemik yapımı birlikte, sklerotik/osteoblastik fazda ise yoğun düzensiz yeni kemik oluşumu baskındır. Paget hastalığı hastalık sürecinde osteolitik, mikst ve sklerotik fazlardan oluşan aşamalı kemik dönüşümü gösterir. Paget hastalığı başlangıçtaki litik, ara mikst ve kronik sklerotik fazlardan oluşur.

Normalde

Normal temporal kemik BT'sinde koklear yarık ile koklear ve vestibüler akuaduktlar gibi normal anatomik kanallar görülebilir. Petroz kemik hacmini, korteks kalınlığını, trabekül kabalığını ve otik kapsül dansitesini aynı kemik penceresinde karşı tarafla karşılaştır; Paget hastalığı tek taraflı veya çoklu kemik tutulumuyla seyredebilir.

Ayırıcı tanı

Fibröz displazi: genişlemiş kemikte homojen buzlu cam matriks ve düzgün dış kontur daha tipiktir.
Otoskleroz: odak çoğunlukla oval pencere önündeki fissula ante fenestram çevresindedir; tüm petroz kemiği genişletmez.
Osteopetroz: yaygın ve daha simetrik kemik yoğunluğu artışı ile kemik iliği boşluklarının daralması beklenir.
Metastatik kemik hastalığı: çok odaklı litik veya sklerotik lezyonlar görülebilir; Paget tipi genişleme ve kaba trabeküler desen tipik değildir.

Tuzaklar

Otik kapsüldeki hipodens alanı otomatik olarak otoskleroz sayma; Paget tutulumu petroz kemiğin genişleme ve kaba trabekülasyonuyla birlikte daha yaygın olur.
Koklear yarık ile koklear ve vestibüler akuaduktlar gibi normal anatomik kanallar temporal kemik BT'sinde patolojiyle karıştırılabilir.
