Özet
Santral dev hücreli granülom çoğunlukla genç erişkinlerde görülen, mandibula ön bölümünü ve bazen orta hattı aşan intraosseöz bir lezyondur. CBCT, lezyonun tek ya da çok odacıklı yapısını, kortikal genişleme veya perforasyonu ve çevre dokularla ilişkisini değerlendirmede etkilidir; tanı klinik, görüntüleme ve histopatolojik bulgularla birlikte konmalıdır. Agresif varyantlar hızlı büyüme, ağrı, kök rezorpsiyonu, kortikal genişleme veya erizyonu ve nüks eğilimi gösterebilir. CBCT, lezyonun yayılımı, iç mimarisi ve kortekse tutulumunun değerlendirilmesinde etkilidir; kortikal erizyon ve kök rezorpsiyonu sıklıkla izlenir.

- Kontrastsız tanısal
- Kontrastsız BT'de litik, genişleyici kitle, iç septalar veya trabeküller ve kortikal incelme/erizyon izlenebilir. Bu kemik lezyonunun değerlendirilmesi için kontrast fazı gerekmez.
- Kemik algoritması tanısal
- Aksiyel, koronal ve sagittal kemik rekonstrüksiyonları lezyon sınırını, ince kalan korteksi, perforasyon alanlarını, trabeküler septaları ve komşu diş köklerinin yer değiştirmesi ya da rezorpsiyonunu ortaya koyar.
Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).
BT bulguları
- Ön mandibulada simfiz veya parasimfizden başlayıp orta hattı geçebilen intraosseöz litik lezyon.
- İç yapısı tek odacıklı olabileceği gibi, ince kemik septalarıyla çok odacıklı görünüm de alabilir.
- Ekspansiyon mandibulada bukkal ve lingual korteksi inceltir; ileri lezyonda kortikal bütünlük bozulabilir.
- Komşu diş köklerinde rezorpsiyon görülebilir; bu özellik agresif lezyonlarla daha sık ilişkilendirilir.
- Lezyonun sınırı iyi seçilebileceği gibi agresif olgularda daha az belirgin ve destrüktif olabilir.
- Maksillada lezyonun sinüs tabanı, burun tabanı ve komşu alveoler çıkıntıya uzanımı çok düzlemli görüntülerde haritalanır.
- Yumuşak dokuya taşan bölüm kortikal açıklıktan devam edebilir; tek başına genişleme yumuşak doku yayılımı anlamına gelmez.
Ölçütler ve sınıflamalar
- Lokal agresif davranış gösterebilen santral dev hücreli granülom
- Agresif varyantlar hızlı büyüme, kortikal genişleme, ağrı, kök rezorpsiyonu ve nüks eğilimi gösterebilir; histopatolojik olarak agresif olgularda dev hücre sayısı ve inflamatuvar infiltrasyonda hafif artış eğilimi görülebilir. BT bu özellikleri gösterir, ancak alt tip kararı klinik ve histopatolojik bilgilerle verilir.
Normalde
Normal ön mandibulada korteks kesintisizdir; trabeküler kemik, simfiz boyunca düzenli yoğunlukta izlenir ve diş köklerinin lamina durası korunur. Koronal ve sagittal çok düzlemli rekonstrüksiyonlar ile 3D görüntüleme, lezyonun yayılımını, iç mimarisini ve kortikal tutulumunu değerlendirerek gerçek ekspansiyonun saptanmasında etkilidir.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Radyolojik ve histopatolojik incelemeler tek başına tanıyı belirlemek için yeterli değildir; ayırım için biyokimyasal testler gerekir.
