Özet

Glandüler odontojenik kist nadir, gelişimsel bir odontojenik kisttir; mandibula ön bölümünde görülme eğilimi ve yerel agresif genişleme potansiyeli nedeniyle tanınması önem taşır. BT, lezyonun uniloküler veya multiloküler yapısını, kortikal incelme ya da perforasyonu ve diş köklerine etkisini haritalar; görüntüleme görünümü özgül değildir. Ameloblastom, odontojenik keratokist ve santral mukoepidermoid karsinom gibi lezyonlarla ayrımı histopatolojiye dayanır. Kortikal perforasyonu veya yumuşak dokuya taşmayı atlamak lezyonun yayılım kapsamının eksik bildirilmesine yol açabilir.

Faz ve pencere

Kontrastsız tanısal
Kontrastsız BT'de mandibula ön bölümünde, bazen orta hattı aşan, iyi sınırlı düşük atenüasyonlu tek ya da çok odacıklı intraosseöz lezyon görülebilir; scalloplu sınır ve bukkal-lingual genişleme eşlik edebilir. Kontrastsız BT veya KBKT, lezyonun kemik yapısını ve kortikal durumunu değerlendirmek için yeterli ve standart yaklaşımdır.
Kemik algoritması tanısal
Kemik rekonstrüksiyonları kortikal tabakanın incelmesini, tek veya birden çok perforasyonu, alt mandibular sınırın yer değiştirmesini ve komşu dişlerde deplasman ya da kök rezorpsiyonunu gösterir; lezyonun multiloküler görünmesi tanıyı tek başına doğrulamaz.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Ön mandibula gövdesinde iyi sınırlı radyolüsent kemik içi lezyon; simfiz ve parasimfiz boyunca uzanarak orta hattı aşabilir.
  • İç yapı uniloküler olabileceği gibi ince septalarla multiloküler ve scalloplu da görünebilir.
  • Bukkal ve lingual kortekslerde genişleme ve incelme; daha ileri lezyonda kortikal perforasyon gelişebilir.
  • Komşu dişlerde yer değiştirme görülebilir; kök rezorpsiyonu da oluşabilir ancak zorunlu değildir.
  • Büyük lezyon mandibulanın alt sınırını inceltebilir veya aşağı doğru yer değiştirebilir.
  • Kortikal perforasyon varsa lezyonun çevre yumuşak dokuyla ilişkisi aranır; BT'de yumuşak doku komponenti değişken seçilebilir.
  • Görüntüleme bulguları özgül olmadığından uniloküler veya multiloküler şekil glandüler histolojiyi kanıtlamaz.

Normalde

Lezyonun bulunduğu bölgedeki trabeküler yapı, korteks kalınlığı ve mandibula alt sınırının konturu kontralateral bölgeyle karşılaştırmalı olarak değerlendirilerek fokal kortikal açıklık veya asimetri doğrulanmalıdır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
