Pathologic mandibular fracture

Eğitim amaçlıdır; hasta başında tanı ve tedavi kararının yerine geçmez.

Özet

Düşük enerjili travma ya da çiğneme sırasında gelişen mandibula kırığında, kırık hattının önceden hastalıklı kemikten geçip geçmediği araştırılır. Çok kesitli BT, kırık geometrisi, fragman deplasmanı ve alttaki litik-sklerotik kemik değişikliğini detaylı göstererek cerrahi planlamayı yönlendirir; kitle veya yumuşak doku yayılımı şüphesi durumunda intravenöz kontrast eklenebilir. Osteoradyonekroz, ilaç ilişkili osteonekroz, osteomiyelit ve tümör başlıca zeminlerdir. Lezyonun kırık nedeni olarak tanınmaması maligniteyi veya nekrotik kemiği atlayabilir ve mekanik instabiliteyi olduğundan az gösterebilir.

Okuma sırası

  1. Mandibula korpusu. BT ve koronal-sagital yüksek çözünürlüklü rekonstrüksiyonlar, kırığın uzamsal ilişkisini ve deplasmanını değerlendirmek için tercih edilir.
  2. Kırık hattı. Üç boyutlu BT rekonstrüksiyonları, kırık hattının komşu anatomik yapılarla ilişkisini ve deplasmanını daha net gösterir.
  3. Kırık çevresi medüller kemik. Kırık hattının geçtiği bölgedeki medüller kemik yapısı, kortikal bütünlük ve altta yatan litik-sklerotik lezyon özellikleri detaylı olarak değerlendirilmelidir.
  4. Mandibula alt kenarı. Çok odaklı kırılmalarda tek taraflı veya çift taraflı fragmentasyon sıklıkla görülür; kırık hattının uzamsal ilişkisi ve fragman deplasmanı incelenir.
  5. Mandibular çevre yumuşak dokular. Son olarak kemik dışındaki kitleyi, gazı ve inflamasyonu arayın; bunlar tümör, nekroz veya enfeksiyon zeminini daraltır.

Kontrastsız tanısal
Kemik algoritmasında kırık çizgisi, kortikal basamaklanma ve medüller litik/sklerotik zemin; yumuşak doku penceresinde eşlik eden şişlik ve gaz aranır. BT incelemesi, kemik algoritması ve yüksek çözünürlüklü rekonstrüksiyonlar kullanılarak kırık hattı ve alttaki kemik lezyonunun karakterize edilmesinde birincil yöntemdir.
Kemik algoritması tanısal
Çok kesitli BT ve koronal-sagital yüksek çözünürlüklü rekonstrüksiyonlar, kırığın uzamsal ilişkisini ve fragman deplasmanını daha iyi kavramayı sağlar.

BT bulguları

  • Hastalıklı kemikten geçen kırık — kırık hattının, altta yatan hastalığa bağlı olarak zayıflamış olan ve litik, sklerotik veya kortikal incelme bulguları gösteren kemik bölgesinden geçmesi.
  • Kortikal destrüksiyon — mandibula alt kenarı ya da bukkal/lingual kortekste düzensiz kesinti ve kemik kaybı.
  • İlaç ilişkili çene osteonekrozunda karışık tipte osteoskleroz, sekestrum ayrışması ve periostal reaksiyonlar görülebilir; bulgular tedavi öyküsüyle birlikte değerlendirilir.
  • Tümöral zemin — permeatif veya coğrafi kemik yıkımına eşlik eden yumuşak doku kitlesi ya da fokal medüller ekspansiyon.
  • Osteomiyelit bulguları — medüller destrüksiyon, periostal yeni kemik ve çevre yumuşak dokuda inflamatuvar değişiklik.
  • Kırık fragmanlarının ayrışması — mandibula korpusunda basamaklanma, angulasyon veya parçalanma ile oluşan mekanik instabilite ve oklüzyon bozukluğu.
  • İnferior alveolar kanal komşuluğu — lezyonun mandibular kanalı genişletmesi, korteksel sınırını eritmesi veya sekestrumun kanala kompresyon yapması nörosensör bozukluklarla ilişkili olabilir.
  • Patolojik mandibula kırıkları, altta yatan sistemik veya lokal durumların kemik bütünlüğünü zayıflatmasıyla gelişir; tanı klinik muayene ve görüntülemeye dayanır.
  • Raporlamada kırığın anatomik yeri, deplasman ve parçalanma özellikleri ile altta yatan patolojinin varlığı belirtilir.

Normalde

Karşı tarafla yan yana değerlendirme, mandibular kemik yoğunluğu ve kortikal kalınlık farklılıklarını ortaya koymaya yardımcı olabilir.

Primer veya metastatik kemik tümörü
ekspansil veya permeatif kemik yıkımı ve yumuşak doku kitlesi ile seyreden, osteonekroz veya osteomiyelit ile klinik ve radyolojik olarak benzeşebilen ancak kesin ayırıcı tanı için biyopsi gerektiren lezyon.

Tuzaklar

  • Patolojik kırık tanısında yalnızca kırık hattına odaklanılmamalı; altta yatan osteolitik, sklerotik veya nekrotik kemik değişikliği ve yumuşak doku bulguları sistematik olarak taranmalıdır.
  • Osteonekrozda sklerozu tümör lehine tek başına yorumlamayın; sekestrum, periostal kemik ve önceki radyoterapi/ilaç öyküsünü birlikte tartın.
