Condylar hypoplasia or hyperplasia

Eğitim amaçlıdır; hasta başında tanı ve tedavi kararının yerine geçmez.

Özet

Kondil hipoplazisi, kondilin ve bazen ramusun yetersiz gelişimini; kondil hiperplazisi ise kondilin aşırı büyümesini ifade eder. Hemimandibular hiperplazide etkilenen tarafta dikey boyut artışı, hemimandibular elongasyonda ise mandibulanın ilerleyici lateral deviasyonu görülebilir. Gelişimsel deformite, eski travma veya aktif asimetrik büyüme klinik olarak yüz ve oklüzyon asimetrisine yol açabilir. BT'de kondil morfolojisi ile etkilenen taraftaki ramus uzunluğu ve genişliği karşı tarafla karşılaştırılabilir. Kondil dışındaki ramus boyutları da morfolojik değerlendirmeye katılmalıdır; kondil büyüme aktivitesinin değerlendirilmesinde kemik SPECT kullanılabilir.

Okuma sırası

  1. Bilateral kondil başları. Koronal kesitte iki mandibular kondil başını aynı anatomik seviyede yan yana bulun; belirgin hacim veya yükseklik farkını not edin.
  2. Kondil boynu. Kondil boynunu aşağı doğru izleyerek kısa, uzun, kalın ya da eğimli segmentin asimetriye katkısını belirleyin.
  3. Mandibular ramuslar. Her iki ramusun kondil tepesi ile mandibular açı arasındaki dikey uzunluğunu kıyaslayın.
  4. Mandibular orta hat ve TME yüzleri. Mentonun orta hattan sapmasını ve gövde/mandibular açı değişikliğini inceleyin; kırık kallusu, eklem erozyonu veya osteofit gibi eşlik eden nedeni arayın.

Kemik algoritması tanısal
İnce aksiyel ve koronal rekonstrüksiyonlar ile 3B kemik görünümünde iki kondil, ramus yüksekliği, mandibular açı ve çene orta hattının asimetrisi değerlendirilir.

  • Kondil başı ve boynunun karşı tarafa göre küçük, yassı, kısa veya hacimce artmış olması

  • Menton ve mandibular orta hattın deviasyonu ile diş arkı düzeyindeki asimetri
  • Kondil eklem yüzünde düzensiz erozyon, skleroz, osteofit ya da eski kırık kallusu gibi alternatif nedenler
  • Karşı kondilin de etkilenip etkilenmediği; iki tarafın boyut ve şekil karşılaştırması
  • Büyüme aktivitesinin değerlendirilmesinde seri görüntüleme ve klinik bulguların zaman içindeki seyri dikkate alınır; uygun olgularda SPECT yardımcı bilgi sağlayabilir.

Ölçütler ve sınıflamalar

Obwegeser–Makek kondil hiperplazisi sınıflaması
Obwegeser–Makek sınıflamasında hemimandibular hiperplazi ve hemimandibular elongasyon ayrı paternlerdir; kondil hiperplazisi etkilenen tarafta dikey boyut artışıyla, elongasyon ise ilerleyici mandibular lateral deviasyonla ilişkilidir. İki patern birlikte görülebilir (hibrit patern). Bu sınıflama büyüme aktivitesini göstermez; tek zamanlı BT aktif büyümeyi kanıtlamaz.

Normalde

Normal karşılaştırmada iki kondil başı, kondil boynu ve ramus konturları kabaca benzer gelişim ve yüksekliktedir; eklem yüzü düzgün kortike görünür. Koronal BT'de ramusların dikey uzunluğunu, aksiyel ve 3B görüntüde kondil hacmi ile mandibular orta hattın yerini kıyaslayın; fizyolojik hafif asimetriyi tek başına hastalık saymayın.

Eski kondil kırığı
boyun konturunda kallus, kısalma veya açılanma ve eklem yüzünde ikincil yeniden şekillenme aranır.
Mandibular kondil osteokondromu
fokal ekzofitik kemik büyümesi ve kortikomedüller devamlılık asimetrik kondil hacminden ayrılır.
TME osteoartriti
kondil başında düzleşme, osteofit, subkondral skleroz ve kortikal erozyon birlikte olabilir.
Mandibular hemifasiyal mikrozomi
kondil küçüklüğüne ramus, mandibular gövde ve bazen orta kulak/zigomatik kemik gelişim anomalileri eşlik edebilir.
Kondil aplazisi
eklem başının tamamen yokluğu, hipoplazideki küçük fakat mevcut kondilden ayrılır.

Tuzaklar
