Özet

Appendiks nöroendokrin tümörü çoğunlukla appendektomi materyalinde tesadüfen tanınır; küçük odak özellikle appendiks ucunda yerleştiğinde BT'de fark edilmeyebilir. Kontrastlı BT'de fokal mural kalınlaşma veya hipervasküler nodül görülebilir ve akut appendisit görünümü eşlik edebilir. BT, çevre yağ dokusunu, bölgesel lenf nodlarını ve ileri hastalıkta karaciğeri değerlendirmeye yardım eder; tümörün varlığı çoğu kez histopatolojiyle kesinleşir. Appendisit görüntüsünü otomatik olarak tümöre bağlamak da, inflamasyon içindeki fokal duvar lezyonunu tamamen göz ardı etmek de uygun değildir.

Faz ve pencere

Portal tanısal
Appendiks duvarında fokal nodüler kalınlaşma veya kontrastlanma, çevre yağ planları, apse ve bölgesel nodlar incelenir; küçük tip lezyonlar seçilemeyebilir.
Arteriyel
Klinik olarak tümör kuşkusu veya evreleme incelemesinde küçük hipervasküler duvar nodülü ve hipervasküler karaciğer odağı daha belirgin olabilir; rutin appendisit BT'sinde şart değildir.

Önerilen pencereler: Batın (G 400 / M 50), Karaciğer (G 150 / M 30).

BT bulguları

  • Appendiks ucunda küçük, solid, duvar kökenli nodül; küçük boyut nedeniyle rutin BT'de görünürlüğü sınırlı olabilir.
  • Appendiks NET'i BT'de nodüler duvar kalınlaşması şeklinde görülebilir; küçük lezyonların BT'de ayırt edilmesi güç olabilir.
  • Appendiks NET'i BT'de küçük submukozal kitle veya nodüler duvar kalınlaşması şeklinde görülebilir ve akut appendisitten ayırt edilmesi güç olabilir.
  • Akut appendisit ile ilişkili dilatasyon ve çevresel yağ çizgilenmesi görülebilir; bu görünüm tümörün inflamasyonun nedeni olduğunu tek başına kanıtlamaz.
  • Daha ileri lezyonda appendiks tabanı/çekum komşuluğu, mezappendiks, çevre yağ dokusu ve ileokolik bölgesel lenf nodları değerlendirilir.

Normalde

Aynı koronal düzeyde appendiks ucunun çekum ve terminal ileumla ilişkisini izle; lümen içi appendikolit ile duvar kökenli solid odağı ayırmak için aksiyel kesitleri de karşılaştır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Appendikolit akut apandisit BT’sinde görülebilir; eşlik eden küçük veya infiltratif NET BT’de seçilemeyebilir.
Akut appendisit
genişlemiş, kontrastlanan duvar ve periappendiküler yağ çizgilenmesi ön plandadır; ayrı solid tip nodülü şart değildir.
Mukosel/LAMN
belirgin lümen genişlemesi ve düşük atenüasyonlu mukusla dolu tübüler yapı; mural nodül farklı neoplazi kuşkusunu artırır.
Appendiks adenokarsinomu
daha infiltratif duvar kalınlaşması, çekum tutulumu veya mukinöz/peritoneal bulgular gösterebilir.
Çekum tümörü
kitle çekum duvarından kaynaklanır ve appendiks tabanını ikincil tıkayabilir; kökeni koronal reformasyonlarda izle.

Tuzaklar

  • Appendiks NET'lerinin çoğu küçük ve uç yerleşimli olduğundan normal veya hafif bulgulu BT tanıyı dışlamaz; histopatoloji belirleyicidir.
  • Appendikolit benzeri yoğun odak tümör nodülü olabilir; lümen merkezinde mi, duvara bitişik mi olduğunu incele.
  • Appendisit eşlik edebilir; tümörü inflamasyonun kesin nedeni olarak sunma ve apse/perforasyon gibi gerçek komplikasyonları ayrıca tarifle.
  • Appendiks ucunu kısmi hacim etkisiyle kaçırmak kolaydır; koronal ve sagital reformatlarla kör ucu baştan sona takip et.
