Özet

Hipofiz metastazı nadirdir; en sık akciğer ve meme kanseri gibi sistemik malignitelerden gelir ve posterior lob, infundibulum veya kavernöz sinüs tutulabilir. BT, özellikle kafa tabanı kemik erozyonunu, sfenoid sinüs/klivus yayılımını ve eşlik eden başka metastatik bulguları gösterebilir; küçük yumuşak doku infiltrasyonu rutin incelemede kaçabilir. Kemik erozyonunun belirgin sella genişlemesi olmadan görülmesi metastaz kuşkusunu artırır, ancak tanı koydurmaz; sellaya yönelik MRI yayılımı daha iyi haritalar. Lezyonun gözden kaçması görme yolu basısını, kraniyal sinir tutulumunu veya kanser yayılımını olduğundan düşük değerlendirmeye yol açabilir.

Faz ve pencere

Kontrastsız
Sellar/parasellar yumuşak doku odağı silik veya izodens olabilir; kemik algoritmasında sellar taban, dorsum sellae, sfenoid sinüs tavanı ve klivusta destrüksiyon ya da erozyon aranır. Hipofiz metastazlarında infundibulum ve posterior lob tutulumunu değerlendirmek için yüksek çözünürlüklü MRI tercih edilir; kontrastsız BT ise sellar kemikteki erozyon ve dejeksiyonu belirlemekte önceliklidir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400), Beyin (G 80 / M 40).

BT bulguları

  • Sellar kemikte odaklanmış erozyon veya destrüksiyon; metastazda kemik kaybı belirgin sella genişlemesi olmadan gelişebilir
  • İnfundibulum boyunca kalınlaşma ya da suprasellar sisternaya uzanan infiltratif yumuşak doku; rutin BT’de küçük odak sınırlı seçilir
  • Posterior hipofiz ağırlıklı yerleşim olasılığı; BT’nin lobları ayırt etme yeteneği sınırlı olduğundan bu dağılım MRI ile netleşir
  • Kavernöz sinüs, klivus veya sfenoid sinüse komşu yumuşak doku uzanımı ve ilgili kemik korteksinde düzensizlik
  • Dumbbell biçimli sellar-suprasellar yumuşak doku, metastatik infiltrasyonda görülebilecek ancak özgül olmayan morfoloji
  • Aynı incelemede klivus, sfenoid kemik ve görüntü alanına giren diğer kafa tabanı kemiklerinde ek metastatik odaklar

Normalde

Normal sella tabanı ve dorsum sellae düzenli kemik konturuna sahiptir; BT, bu bölgelerdeki ince kemik detaylarını ve erozyon/destrüksiyon gibi agresif bulguları değerlendirmek için seçilen yöntemdir. Aynı seviyede kemik korteksinin erozyonunu, sellanın büyüyüp büyümediğini ve klivus/sfenoid sinüs komşuluğunda infiltratif doku olup olmadığını karşılaştır; düzgün genişleme ile küçük odaklı kemik kaybını ayır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Plazmasitom
klivusta litik kemik lezyonu oluşturabilir; kemik merkezli kitle ve ek iskelet odakları aranır.
Lenfoma
infundibulum veya kavernöz sinüs infiltrasyonu yapabilir; sellar metastazdan BT ile kesin ayrım çoğu zaman yapılamaz.
Hipofiz apopleksisi
mevcut adenoma içine akut kanama sellar kitle ve ani semptom oluşturur; klinik başlangıç ve MR kan ürünleri ayırıcıdır.

Tuzaklar

  • Kanser öyküsü metastazı destekler ama her sellar kitleyi metastaz yapmaz; PitNET, meningiom ve vasküler taklitçileri anatomik bulgularla sınamayı sürdür.
  • Metastaz posterior lob veya infundibulumu tutabilir; rutin aksiyel BT’de küçük stalk lezyonunun seçilmemesi tutulum yokluğunu göstermez.
  • Kafa tabanı erozyonu PitNET ve enfeksiyonlarda da görülebilir; tek başına kemik destrüksiyonuyla histolojik köken belirlenemez.

    Hipofiz metastazı. Kanser öyküsü, stalk çevresi infiltrasyon ve sellar genişleme olmadan kemik erozyonu metastaz kuşkusunu artırır. Makroadenom daha çok sellayı genişletip remodel eder; meningiom dural taban/hiperostoz, Rathke kisti orta hat kistik içerik gösterir.

  2. Sellar odağın klivusla ilişkisi belirsiz. Hangi BT bulgusu kordomayı metastatik sellar odaktan daha olası kılar?

    Klivus merkezli litik yıkım. Klivus merkezli, orta hat litik destrüksiyon kordoma için tipik başlangıç yerini gösterir ve kordomayı destekler. Posterior hipofiz/infundibulum tutulumu ve infundibulum kalınlaşması hipofiz metastazında görülebilir; kafa tabanında çok odaklı litik lezyonlar da metastaz lehine bir dağılımdır. Posterior hipofiz lobu ve küçük infundibulum odaklarını BT sınırlı değerlendirebilir; MRI bu yumuşak doku ilişkilerini daha iyi gösterir.
