Özet

Bezoar, sindirilemeyen materyalden oluşan intralüminal bir kitle olup ince barsak tıkanmasına neden olabilir; önceki mide cerrahisi ve mide motilitesindeki değişiklikler bezoar oluşumuyla ilişkili klinik etkenlerdir. BT’de geçiş noktasındaki oval, benekli gaz içeren intralüminal kitleyi proksimal dilatasyon ve distal kollapsla birlikte aramak gerekir. Küçük barsak dışkısı bulgusunu tıkayıcı bezoarla karıştırmamak gerekir; bu bulgu obstrüksiyonun geçiş noktasını lokalize etmeye yardımcı olabilir, ancak altta yatan nedeni belirlemez.

Faz ve pencere

Portal tanısal
Geçiş noktasındaki intralüminal mottled-gazlı kitle, komşu duvarın kontrastlanması, proksimal ve distal ans kalibresi, mezenterik ödem ve sıvı değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Geçiş noktasında iyi sınırlı, oval veya yuvarlak intralüminal yumuşak doku yoğunluklu kitle içinde benekli gaz odakları.
  • Kitle düzeyinde ani kalibrasyon değişikliği; proksimal ince barsak ansları geniş, distal anslar kollabe olabilir.
  • Mide veya başka bir barsak segmentinde eşlik eden bezoar benzeri intralüminal kitle, bezoar tanısını destekler.
  • Bezoarın proksimalindeki dilate ince barsak ansında gazla karışık partiküler içerik küçük barsak dışkısı bulgusunu oluşturabilir; bu özgül olmayan bulgu geçiş noktasını lokalize etmeye yardımcı olur, ancak obstrüksiyonun nedenini göstermez.
  • Birden fazla ince barsak bezoarı aynı hastada birlikte bulunabilir.
  • İnce barsak obstrüksiyonunda mezenterik ödem, peritonit ve asit nekrozla ilişkili olabilir; kapalı ans bulguları da nekrozla ilişkili görüntüleme bulguları arasındadır.

Normalde

Normal ince barsak lümeni homojen sıvı, gaz veya kontrastla izlenebilir; geçiş noktasında sınırlı gaz benekli bir kitle ve bunun iki yanında ani kalibre farkı bulunmaz. Kıyaslamada şüpheli içeriğin uzun bir dilate segment boyunca mı dağıldığını, yoksa tıkanma seviyesinde ayrı ve sınırlı bir oval kitle olarak mı durduğunu belirle.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
