Özet

Duodenal divertikül, en sık duodenumun ikinci bölümünün medial duvarından pankreas başına komşu olarak dışa uzanan ve çoğu kez rastlantısal görülen bir keseciktir. BT, hava, sıvı, debris veya oral kontrast içerebilen keseyi lümenle bağlantısı üzerinden tanımlar; çevre inflamasyonu da komplikasyonu gösterir. Divertikülit, akut üst karın ağrısında akut kolesistit, akut pankreatit veya peptik ülser hastalığını taklit edebilir. Perforasyon, apse ya da periampuller basıya bağlı biliyer obstrüksiyonun atlanması klinik kötüleşmeye yol açabilir.

Faz ve pencere

Portal tanısal
İntravenöz kontrastlı portal fazda medial duodenal duvardan, çoğunlukla ikinci bölüm çevresinden çıkan kese; içinde hava, sıvı-hava seviyesi, debris veya oral kontrast ve lümene bağlanan boyun aranır. Divertikülitte çevre duvar kalınlaşması/kontrastlanması, retroperitoneal yağ dokusunda kirlenme, sınırlı koleksiyon ya da ekstraluminal gaz görülebilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Medial duodenal kese — ikinci veya üçüncü bölümün medial yüzünden pankreas başına doğru uzanan düzgün sınırlı sakküler çıkıntı tipik yerleşimdir.
  • Lümen bağlantısı — divertikül boynunun duodenumla devamlılığını aksiyel ve koronal kesitlerde göstermek, sıvı içerikli kistik taklitçileri ayırır.
  • Değişken içerik — hava, sıvı, hava-sıvı seviyesi, besin artığı veya oral kontrast aynı divertikülde görülebilir.
  • Divertikülit — kese duvarında kalınlaşma/kontrastlanma ve çevresindeki periduodenal ya da retroperitoneal yağda inflamatuvar çizgilenme bulunur.
  • Flegmon veya apse — divertikül çevresinde sınırlı yumuşak doku veya sıvı koleksiyonu, içinde gaz kabarcıklarıyla belirginleşebilir.
  • Sınırlı perforasyon — kese çevresinde fokal ekstraluminal hava, özellikle divertikül boynuna komşu olduğunda perforasyon odağını gösterebilir.
  • Serbest retroperitoneal hava — ikinci ve üçüncü duodenum bölümleri retroperitoneal seyreder; sağ ön pararenal boşluktaki hava duodenal perforasyonla uyumlu olabilir.
  • Periampuller bası — papilla yakını divertikül ana safra kanalını sıkıştırabilir; safra yolu genişlemesi, kolanjit veya pankreatit eşlik edebilir.

Normalde

Normal duodenumun ikinci bölümünün medial konturu pankreas başına düzgün komşuluk gösterir; buradan lümen dışına uzanan hava-sıvı içeren kese veya çevre yağda çizgilenme bulunmaz. Aynı düzeyde duodenal lümeni baştan sona takip edip olası divertikül boynunun lümene açıklığını ve pankreas başı, distal koledok ile periduodenal yağın görünümünü kıyaslayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
