Özet

İnce barsak dışkı işareti, lümendeki partiküllü içerik ile küçük gaz kabarcıklarının kolonik dışkıyı andıran görünümüdür ve yavaş transit ya da stazı yansıtır. Mekanik ince barsak obstrüksiyonunda çoğu kez geçiş noktasının hemen proksimalinde yer alarak geçiş yerini bulmaya yardım eder. İşaretin varlığı tek başına obstrüksiyon kanıtı değildir; bağırsak kalibresi değişimi ve distal dekompresyonla birlikte yorumlanmalıdır. Onu tek başına obstrüksiyon sanmak yanıltıcı olabilir; ayrıca semptomatik olmasa bile iskemiyle ilişkili olabileceği unutulmamalıdır.

BT bulguları

• Staz nedeniyle ince bağırsak lümeninde dışkı benzeri partiküllü içerik ve gaz kabarcıkları görülür.
• Geçiş noktasına yakın yerleşim — obstrüksiyon varsa fekalize içerik sıklıkla dilate proksimal ansın distal ucunda, kalibre değişiminin hemen önündedir.
• Proksimal ans genişlemesi — tıkanma öncesi segmentlerde lümen ve ans kalibresi artar; tek başına fekalizasyon bu kriterin yerini tutmaz.
• Ani kalibre değişimi — dilate ansın daralarak kollabe distal ince barsağa geçtiği odak mekanik geçiş noktasını işaret eder.
• Distal dekompresyon — geçiş noktasının gerisindeki ince barsak anslarında lümen içeriği ve çap azalır.
• Kompresyon veya neden — geçiş noktasında adezyona bağlı düzgün angülasyon, herni, kitle, striktür ya da kapalı ans bulgusu araştırılır.
• İşaretin obstrüksiyonsuz görülmesi — anlamlı ans dilatasyonu ve belirgin kalibre geçişi yoksa fekalizasyon stazı veya yavaş transit ile ilişkili olabilir.

Normalde

Normal ince barsak lümeni ince sıvı veya gazla dolu olabilir; kolondaki gibi yoğun partiküllü dışkı benzeri materyal ve gaz kabarcıkları ardışık bir ans içinde belirginleşmez. Şüpheli segmentin proksimal ans çapını, geçiş noktasındaki ani daralmayı ve distal ince barsak dekompresyonunu değerlendirin.
