Özet

İnce barsak lenfoması, sistemik lenfomanın barsak tutulumunun parçası olabileceği gibi primer ekstranodal hastalık şeklinde de görülebilir. Kontrastlı batın BT, duvar tutulumunun uzunluğunu ve şeklini, mezenterik/retroperitoneal nodları, diğer organları ve perforasyon ya da obstrüksiyon gibi acil komplikasyonları gösterir. Lenfoma kas tabakasını ve miyenterik pleksusu infiltre ederek lümeni daraltmadan genişletebilir; bu görünüm, diğer birçok daraltıcı barsak tümöründen ayrılmasına yardım eder. Bununla birlikte her lenfoma anevrizmatik değildir ve eşlik eden obstrüksiyon, invajinasyon veya perforasyon gelişmesi klinik önceliği değiştirir.

Faz ve pencere

Portal tanısal
Homojen kontrastlanan sirkumferansiyel veya nodüler duvar kalınlaşması, polipoid kitle ve mezenterik nodlar araştırılır. Lümenin tümör segmentinde açık kalıp genişlemesi ile proksimalde gerçek geçiş noktası ve ans dilatasyonu birbirinden ayrılır; nekroz, çevre yağ planı ve serbest hava da bu fazda değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Uzun segment boyunca sirkumferansiyel, homojen duvar kalınlaşması; tutulan ansın kıvrımlarını silip lümende düzgün ya da düzensiz genişleme bırakabilir.
  • Anevrizmal lümen genişlemesi — infiltrasyonun kas tabakasını ve miyenterik pleksusu bozmasıyla tümör segmenti daralmak yerine geniş kalır.
  • Polipoid veya nodüler intralüminal kitleler tek ya da çok odaklı olabilir; ülserasyon ve lümen içeriğiyle kısmi örtüşme eşlik edebilir.
  • Ekzenterik uzanımlı kitle, komşu ansları birbirinden ayırabilir veya çevre mezenter yağ dokusuna ilerleyebilir.
  • Mezenterik ve retroperitoneal lenf nodları, barsak duvarı lezyonuyla birlikte değerlendirilir; lenfomalarda nodlar genellikle nekroz içermeyen düzgün büyüklükte olur, bu özellik adenokarsinomdan ayırıcı tanıya yardımcı olabilir.
  • İleri ülserasyon, kaviter duvar defekti veya fistül komşu ansla anormal bağlantı oluşturabilir.
  • Belirgin proksimal ince barsak genişlemesi ve keskin geçiş noktası lenfomada daha az tipiktir ama olabilir; segmentin açık lümenli görünmesi obstrüksiyonu dışlamaz.
  • Serbest intraperitoneal hava, ince barsak lenfomasında perforasyon kuşkusunu artırır.

Normalde

Aynı seviyedeki komşu ansları ve mezenterik nodları referans alarak duvar simetrisini, lümen çapının tutulan segment içinde artıp artmadığını ve proksimalde gerçek bir geçiş noktası oluşup oluşmadığını kıyasla.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

İnce barsak adenokarsinomu
kısa ve asimetrik daraltıcı kitle, omuzlaşan kenarlar ve proksimal obstrüksiyon daha çok beklenir.
Crohn hastalığı ile lenfoma ayrımında, lenfomanın daha az segmenti tutma ve daha kalın duvar oluşturma eğilimi yardımcı olabilir.
Gastrointestinal stromal tümör
çoğu kez ekzofitik, iyi sınırlı ve heterojen kitle biçimindedir; uzun segment homojen infiltrasyon ve yaygın nodlar daha az tipiktir.
Metastatik ince barsak hastalığı
bilinen primer tümör, serozal/peritoneal nodüller veya çoklu polipoid odaklar dağılımı açıklar; histoloji olmadan örtüşme sürebilir.
Enfeksiyöz enterit, ince barsak duvar kalınlaşmasında lenfomanın ayırıcı tanısında yer alabilir.

Tuzaklar

  • Anevrizmatik genişleme görülmemesi lenfomayı dışlamaz; polipoid, infiltratif, ülseratif ve kaviter formlar da vardır.
  • Geniş tümör segmenti lümen açıklığını koruduğu için obstrüksiyon gözden kaçabilir; yalnız tümör içindeki çapı değil, proksimal ansları ve geçiş noktasını da takip et.
  • Uzun segment duvar kalınlaşmasını yalnız Crohn hastalığına bağlama; nodal dağılım, kitle hacmi ve segmentteki lümen davranışı lenfoma olasılığını değiştirebilir.
  • Nekrotik nod veya duvar ülserasyonu varsa küçük perforasyon serbest hava miktarı az olsa da eşlik edebilir; mezenterik yağ ve komşu ansları dikkatle incele.

Adenokarsinom ve fibrostenotik Crohn hastalığı lümen daralması ve obstrüksiyona yatkındır; lenfoma ise kas tabakasını ve miyenterik pleksusu infiltre ederek lümeni genişletebilir (anevrizmatik dilatasyon).
