Özet

Erişkinde semptomatik ince barsak invajinasyonu çoğunlukla polip, tümör, Meckel divertikülü veya başka bir çekici odakla ilişkilidir; BT’de bir ansın komşu ans içine girdiği gösterilir. IV kontrastlı BT, iç içe duvarları ve araya taşınan mezenteri ortaya koyarken olası çekici odağı, proksimal tıkanmayı ve perfüzyon bozukluğunu araştırır. Çekici odaklı ve obstrüksiyonlu invajinasyon strangülasyon, iskemi veya perforasyonla sonuçlanabileceğinden yalnız hedef görünümünü tanıyıp nedenini aramamak klinik açıdan önemli bir hatadır.

Faz ve pencere

Portal tanısal
İç içe geçmiş barsak duvarlarının kontrastlanması, invajinasyon içindeki mezenter damarları/yağı, çekici odak, proksimal dilatasyon ve duvar canlılığı değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Hedef işareti — aksiyel kesitte iç içe geçmiş barsak duvarları eşmerkezli halkalar oluşturur.
  • Sosis biçimli kitle — uzun eksende iç içe geçmiş invajinasyon segmenti boyunca uzanan yumuşak doku görünümüdür.
  • İntussuseptum ile intussusipiens arasında taşınmış mezenter yağı ve eşlik eden damar dalları seçilir.
  • İntralüminal ya da duvara bağlı çekici odak; polipoid lezyon, lipom veya tümör olabilir ve kontrastlı kesitlerle aranmalıdır.
  • İnvajinasyonun proksimalinde genişlemiş, distalinde kollabe anslar ve keskin geçiş mekanik obstrüksiyonu destekler.
  • Mezenterik ödem, invagine segmentte duvar kalınlaşması, sıvı ve duvar kontrastlanmasında azalma venöz konjesyon/iskemi şüphesini artırır.
  • Serbest hava, fokal duvar kesintisi veya komşu koleksiyon perforasyon komplikasyonunu düşündürür.

Ölçütler ve sınıflamalar

Yerleşime göre barsak invajinasyonu
Enteroenterik invajinasyon ince barsakla sınırlıdır; kolokolik invajinasyon kalın barsağı tutar; enterokolonik invajinasyon ince barsak ile kolonu birlikte içerir ve ileoçekal ya da ileoçekokolik olarak tanımlanabilir. Erişkin BT sınıflamasında temel ayrım yerleşim ve çekici odağın varlığıdır.

Normalde

Normal ince bağırsakta bir segment komşu bağırsak segmentinin lümenine ilerlemez; bu iç içe geçme invajinasyonu tanımlar. Hedef görünümünde halkaların ayrı ans kesitleri değil, birbirinin içine ilerlemiş duvar katları olduğunu ve mezenter yağının bu katlar arasında sürüklendiğini doğrula.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

  • Barsak duvarı ödemi ve venöz konjesyon nedeniyle çekici odak amorf veya zıtlaşabilir; invajinasyonun ucunu ve taşınmış mezenter yağını detaylı inceleyerek kitleyi ayırt etmeye çalış.

Çekici odağı olmayan invajinasyonlar geçici, kendini sınırlayan ve obstrüksiyon yapmayan özellikte olma eğilimindedir. Gaz içeren benekli bir lümen içi kitle geçiş noktasında görülürse, bunu otomatik olarak fekalize içerik saymamak; bezoar veya bitkisel yabancı cisim olasılığını düşünmek gerekir.
