Özet

İnce barsak gastrointestinal stromal tümörü, barsak duvarından gelişen ve sıklıkla lümen dışına doğru büyüyen mezenkimal neoplazidir. Kontrastlı BT, kitleyi barsak kökenine bağlamaya, ekzofitik-intramural uzanımı tanımlamaya ve kanama, nekroz, perforasyon ya da metastatik yayılımı göstermeye yarar. Küçük lezyonlarda barsak distansiyonu yetersizse odak gözden kaçabilir; CT enterografi küçük ince barsak tümörlerinin algılanmasına katkı sağlar. Kitlenin barsak duvarına bağlandığı noktanın veya karaciğer ve peritoneumdaki yayılımın gözden kaçırılması, hastanın doğru evrelenmesini ve komplikasyonların (rüptür gibi) fark edilmesini engeller.

Portal tanısal
Portal venöz kontrastlı BT'de jejunum veya ileum duvarıyla devamlı, intramural ya da ekzofitik yumuşak doku kitle izlenebilir; kontrastlanma homojen veya heterojen olabilir, büyük kitlelerde nekroz, kistik dejenerasyon, kanama, kavite ya da lümene fistül eşlik edebilir.
Kontrastsız
Kontrastsız kesitlerde kitle kas düzeyinde yumuşak doku atenüasyonunda olabilir; kanama veya kalsifikasyon görülebilir, ancak canlı tümör kontrastlanması ve metastaz karakterizasyonu yapılamaz.

BT bulguları

  • Jejunum veya ileumla devamlılığı gösterilebilen, lümen dışına büyüyen düzgün konturlu kitle
  • Daha küçük odaklarda intramural veya endolüminal yumuşak doku kabarıklığı
  • Tümör boyutu arttıkça heterojen kontrastlanma, santral nekroz, kistik dejenerasyon ya da intratümöral kanama
  • Ülserasyon ve barsak lümeniyle ileti düşündüren intratümöral gaz veya hava-sıvı seviyesi
  • Kitleye komşu ince barsak ansında bası, lümen daralması veya obstrüksiyon
  • Tümör rüptürü, hemoperitoneum veya serbest hava
  • GIST'te lenf nodu metastazı nadirdir. Evreleme değerlendirmesinde karaciğer ve periton öncelikle incelenir.

Normalde

Normal jejunum ve ileum duvarı ince, lümeni açık ve komşu mezenterden ayrı seçilir; fokal ekzofitik yumuşak doku veya kitleye bağlı ans yer değiştirmesi görülmez. Şüpheli kitlenin duvarla temas ettiği noktayı ardışık aksiyel kesit ve koronal MPR'de izle; duvar devamlılığını, lümene uzanımı, mezenter damarlarıyla ilişkisini ve karaciğer/periton yüzeylerini kıyasla.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

İnce barsak adenokarsinomu
sıklıkla kısa segmentli duvar kalınlaşması ve omuzlanmalı stenoz yapar; BT'de nodüler kitle veya ülseratif lezyon olarak da görülebilir, ekzofitik büyüme GIST için daha tipiktir.
İnce barsak lenfoması
uzun segmentli duvar tutulumu, anevrizmatik lümen genişlemesi veya çoklu nodlar görülebilir.
Nöroendokrin tümör metastazı
hipervasküler mezenterik kitle ve desmoplastik çekinti primer odaktan daha belirgin olabilir.
İnce barsak metastazı
özellikle hipervasküler primer öyküsünde, birden fazla odak veya belirgin kontrastlanan intramural nodüller yapabilir.
Mezenterik desmoid
mezenterde kitle oluşturur, ancak barsak duvarından köken aldığı izlenimi ve lümenle bağlantısı yoktur.

  • Ekzofitik kitlenin barsak kökenini gözden kaçırma; tümörün ansla birleştiği duvar noktasını çok düzlemli kesitlerde takip et.
  • Heterojen kitlenin santral nekrozunu yalnız sıvı koleksiyonu sanma; çevresel canlı tümör dokusunu, lümene olası iletiyi ve kanamayı kontrol et.
