Özet

İnce barsak metastazları hematolojik yayılım, lokal uzanım veya intraperitoneal ekimle gelişebilir; melanom ince barsağa metastaz yapan en sık kanserdir, akciğer ve meme kanserleri de kaynak olabilir. Kontrastlı BT, fokal mural veya polipoid lezyonları, serozal implantları ve bunların yol açtığı invajinasyon, kanama ya da obstrüksiyonu aynı incelemede gösterebilir. Tek bir metastazın polipoid baş noktası gibi davranması ile çok odaklı duvar ve peritoneal hastalık farklı yayılım paternlerini düşündürür, ancak görüntüleme primer histolojiyi tek başına kanıtlamaz. Bilinen malignitesi olan hastada küçük barsak lezyonunu atlamak, kanama veya akut mekanik komplikasyonun nedenini açıklanamamış bırakabilir.

Faz ve pencere

Portal tanısal
Metastatik ince barsak hastalığı BT'de çoklu polipoid intralüminal lezyonlar veya fokal duvar kalınlaşması şeklinde görülebilir. Kontrastlı BT'de ince bağırsak metastazları çoklu polipoid intralüminal lezyonlar veya fokal duvar kalınlaşması şeklinde görülebilir; karındaki diğer metastatik hastalık bulguları da araştırılır.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Melanom metastazları ince bağırsakta polipoid nodüller şeklinde görülebilir ve invajinasyona yol açabilir.
  • Fokal, düzensiz veya nodüler duvar kalınlaşması metastatik infiltrasyonu düşündürür; bilinen primerin tipi ve önceki görüntüler dağılım yorumunu güçlendirir.
  • Çok sayıda intralüminal nodül farklı ince barsak segmentlerinde aynı anda bulunabilir; tek primer tümör gibi görünen kesit, tüm ince barsağın taranmasını gerektirir.
  • Serozal yüzeyde veya mezenterik yüzde implantlar, ans dış konturunda nodüler çekinti ve komşu barsak yüzeylerinde düzensizlik yapabilir.
  • İnvajinasyon — iç içe geçmiş konsantrik barsak duvarları ve araya çekilmiş mezenter yağı/vasküler yapılar hedef ya da sosis görünümü oluşturur; baş noktadaki kitle aranır.
  • Obstrüksiyonda metastatik odak düzeyinde geçiş noktası, proksimal ince barsak dilatasyonu ve distal kollabe anslar birlikte görülür.
  • Lezyon çevresindeki serbest sıvı, duvar kontrastlanma azalması veya mezenterik ödem invajinasyona eşlik eden venöz konjesyon ya da iskemi kuşkusunu artırır.
  • Melanomda karaciğer ve akciğerler sık etkilenen organlardandır; peritoneum da metastatik tutulum gösterebilir.

Normalde

Küçük boyutlu ince bağırsak maligniteleri, normal bağırsakla benzer kontrastlanabildiğinden BT'de normal anslarla karıştırılabilir. BT'de tüm ince bağırsak anslarını dikkatle incele; küçük bağırsak maligniteleri normal anslarla benzer kontrastlanma gösterebilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Primer ince barsak adenokarsinomu
çoğunlukla düzensiz annüler daraltıcı lezyon ve proksimal geçiş noktası oluşturur; bilinen primer malignite ve çoklu polipoid odaklar metastazı destekler.
İnce barsak lenfoması
uzun segment homojen duvar kalınlaşması ve anevrizmatik lümen genişlemesi yapabilir; mezenterik nodal hastalık eşlik edebilir.
Gastrointestinal stromal tümör
genellikle tek, ekzofitik büyüyen ve büyükse heterojen kitle şeklindedir; serozal implant veya diğer organ metastazları varsa ayrım zorlaşabilir.
İnce barsak lipomu
homojen yağ atenüasyonlu submukozal baş noktasıdır; metastatik yumuşak doku kitlesi yağ yoğunluğu göstermez.
Peutz–Jeghers hamartomatöz polipleri
sendromik öyküye eşlik eden çok sayıda polip ve histolojik hamartom yapısı ile metastatik yumuşak doku nodüllerinden ayrılır.

Tuzaklar

  • Melanom öyküsü uzak geçmişte olsa bile küçük barsak metastazı görülebilir; ince barsakları yalnız akut yakınma olan bölgede değil tüm tarama boyunca gözden geçir.
  • İnvajinasyonla birlikte polipoid ince bağırsak lezyonu görüldüğünde, metastatik melanom olasılığını göz önünde bulundur.
  • Kısa segmentli mural metastaz, peristaltik kontraksiyon veya kollabe ansı taklit edebilir; lezyonun ardışık kesitlerde devamını ve komşu düzlem karşılığını kontrol et.
  • Serozal implantlar barsak lümenindeki polipten farklı bir büyüme yüzeyi izler; duvarın dış konturunu ve mezenterik yüzü ayrıca takip et.
