Özet

Portal ve mezenterik venlerde gaz, barsak iskemisi için önemli bir uyarı işareti olsa da, benign nedenlerle de görülebildiğinden kesin tanı için klinik tablo ve laboratuvar sonuçlarıyla birlikte değerlendirilmelidir. Kontrastlı BT gazın venöz dağılımını, olası mezenterik damar nedenini ve eşlik eden barsak canlılığı bulgularını aynı incelemede gösterir. Gaz tek başına transmural nekrozu kanıtlamaz; klinik bağlamı ve ans duvarı görünümünü birlikte tartmak gerekir. İskemi, perforasyon veya sepsis eşlik ediyorsa bulguyu gözden kaçırmak hızla ağırlaşabilecek barsak hasarının tanınmasını geciktirebilir.

Venöz fazda mezenterik venlerde tübüler veya dallanan gazı ve karaciğer periferinde çizgisel, dallanan gazı ara; bağırsak duvarı kontrastlanmasını, damar trombüsünü ve pnömatozisi de değerlendir.
Kontrastsız
Kontrastsız BT'de mezenterik damarlarda ve karaciğer periferinde dallanan gaz seçilebilir; damar açıklığını ve bağırsak duvarı kontrastlanmasını değerlendirmek için kontrastlı BT gerekir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40), Akciğer (G 1500 / M -600).

  • Karaciğerin periferik intrahepatik portal ven dallarında ince, dallanan gaz çizgileri
  • Mezenter kökünden barsak kenarına uzanan venöz arkad ve küçük dallarda gaz kabarcıkları
  • Karaciğer içinde santral yerleşimli dallanıcı hava yerine periferik portal dağılım
  • Gazla ilişkili anslarda duvar içi lineer veya kabarcıklı pnömatozis
  • İskemik anslarda azalmış ya da kaybolmuş duvar kontrastlanması ve lümen distansiyonu
  • Mezenterik ven trombozu, mezenterik ödem ve bağırsak ansları arasında sıvı
  • Serbest intraperitoneal hava, ekstraluminal kontrast kaçağı veya peritoneal sıvı

Normalde

Normal portal ven dalları karaciğer içinde hilustan perifere doğru düzenli biçimde incelir ve lümenlerinde gaz bulunmaz; kontrastlı incelemede mezenterik venler de kesintisiz opaklaşır. Şüpheli incelemede gazın santral safra yollarında mı yoksa periferik portal dallarda mı olduğunu, ayrıca mezenterik ven arkadlarıyla karaciğer dallarının aynı dağılımı sürdürüp sürdürmediğini takip et.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

  • Kontrastsız kesitte mezenterik damardaki gazı küçük lümen gazı ile karıştırma; koronal ve ardışık aksiyel kesitlerde damar sürekliliğini izle.
