Özet

Apendiks mukoseli, lümeninde müsin birikmesiyle genişlemiş apendiksi tarif eden radyolojik-morfolojik bir terimdir; altta yatan neden obstrüksiyon olabileceği gibi müsinöz neoplazi de olabilir. Portal venöz fazlı kontrastlı BT, apendiksin çekumla bağlantısını, duvarını ve peritoneal yayılımı aynı incelemede göstererek cerrahi planlama açısından önem taşır. Duvar düzensizliği, mural yumuşak doku ve çevre doku invazyonu neoplazi şüphesini artırır, ancak benign ve neoplastik nedenleri kesin ayırmaz. Rüptür ve peritoneal müsin gözden kaçarsa psödomiksoma peritonei bulguları eksik evrelenebilir.

Faz ve pencere

Portal tanısal
Kontrastlı portal venöz kesitlerde çekum tabanından çıkan, kör sonlanan kistik-tübüler apendiks ve duvarın düzenliliği değerlendirilir; lümen genellikle sıvı/müsin atenüasyonundadır, mural nodül veya düzensiz duvar kontrastlanması aranır. Peritoneal implantlar, loküle müsin ve karaciğer-dalak yüzeyindeki bası çentikleri de bu fazda taranır.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Çekum tabanıyla kesintisiz izlenebilen, kör sonlanan ve sıvı/müsin içerikle genişlemiş apendiks.
  • Lümen içinde çoğunlukla homojen düşük atenüasyon; yoğun içerik varsa proteinli müsin veya debris olasılığı.
  • Duvar boyunca ince, düzgün kontrastlanma; duvar kalınlaşması ve çevre yağ kirlenmesi süperenfeksiyon veya eşlik eden apandisiti düşündürür.
  • Duvarın çevresel ya da noktasal kalsifikasyonu; tek başına benign-neoplastik ayrımı yaptırmaz.
  • Duvar konturunda asimetrik kalınlaşma veya kontrastlanan mural yumuşak doku nodülü; invaziv tümör şüphesini artırır.
  • Apendiks çevresinde kesintiye uğramış duvar, komşu loküle koleksiyon veya peritoneal boşlukta müsin ile rüptür bulguları.
  • Peritoneal müsin/implantlar, omental nodüler kalınlaşma ve karaciğer ya da dalak yüzeyinde scalloping; psödomiksoma peritonei dağılımı.

Mukosel, müsinle genişlemiş apendiksi tanımlayan radyolojik-morfolojik terimdir ve histolojik tanı değildir. WHO 6. baskı, apendikste müsinöz adenomu ile apendiks müsinöz neoplazilerini (LAMN/HAMN) ayrı tanı varlıkları olarak ele alır; kesin histolojik alt tip ise ancak patolojik incelemeyle belirlenir. Görüntüleme histolojik alt tipi tek başına belirlemez.

Normalde

Normal apendiks ince duvarlı, kör sonlanan ve çekumdan çıkan tübüler bir yapı olarak izlenir; lümeni belirgin biçimde sıvıyla genişlemiş değildir ve çevresinde inflamatuvar yağ değişikliği bulunmaz. Kıyaslarken apendiksin dış çapından çok, çekumla devamlılığına, lümen genişlemesine, duvarın düzgünlüğüne ve peritoneal yüzeylerde müsin/implant olup olmadığına bak. Mukosel çap eşiği tek başına neoplaziyi kanıtlamaz.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Akut apandisit
çevresel duvar kalınlaşması ve belirgin periappendiküler yağ inflamasyonu öne çıkar; kronik müsin birikimi biçiminde düzgün kistik dilatasyon tipik değildir.
Apendikolitli obstrüktif apandisit
lümen tıkanma düzeyinde yüksek atenüasyonlu taş ve distal inflamatuvar genişleme görülebilir.
Çekum tümörü
çekumdan kaynaklanan kitle apendiks kaynaklı kistik lezyonu taklit edebilir; duvar invazyonu, düzensiz sınırlar ve bölgesel lenfadenopati ayırt etmede yardımcıdır.
Meckel divertikülü
distal ileumdan çıkar; çekum tabanıyla bağlantısı yoktur.
Mezenterik kist veya enterik duplikasyon kisti
apendiks lümeniyle devamlılık göstermez ve genellikle bağırsak ansları arasında yer alır.

Tuzaklar

  • Genişlemiş apendiksin dış çapı mukosel lehine olsa da histolojik neoplazi tipini göstermez; raporda morfolojik şüpheyi belirt, kesin histoloji atama.
  • Duvar kalsifikasyonu neoplaziyi tek başına kanıtlamaz; duvar nodülü, kontur düzensizliği ve peritoneal bulgularla birlikte tart.
  • Küçük peritoneal müsin düşük atenüasyonlu serbest sıvı sanılabilir; bağırsak anslarını yer değiştirmesi ve solid organ yüzeylerinde çentiklenmeyi ara.
  • Apendiksin kesitlerde izlenememesi kitle yokluğu demek değildir; koronal planda çekum tabanından uca kadar devamlılığı takip et.
