Özet

apendikolit, apendiks lümeninde oluşan fokal yoğun içeriktir ve akut apandisite eşlik edebilir. Apendiks BT'sinde kalsifik odağın lümen içindeki konumu, apendiks duvarı ve çevresindeki inflamasyondan ayrı ayrı değerlendirilmelidir. Kontrastsız BT, apendikolitin saptanmasında kontrastlı BT’ye eşdeğerdir; apendikolitin tek başına varlığı apandisit için güvenilir bir gösterge değildir. Perforasyonda lümen dışına taşınmış apendikolit, inflamatuvar odağın kaynağını işaret edebilir.

Faz ve pencere

Kontrastsız tanısal
Apendiks lümeninde hiperdens intralüminal kalsifikasyon görülür; odağın lümen içindeki veya dışındaki konumu değerlendirilir.
Portal tanısal
Portal venöz fazda apendiks duvarı ve çevre inflamasyonu değerlendirilir; bazı appendikolitler bu fazda kontrastsız görüntülere göre daha az görünür olabilir. Odağın görülmemesi apandisiti dışlamaz; duvar ve periappendiküler bulgular ayrıca incelenir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Apendiks lümeninde BT ile görülen yüksek dansiteli fokal kalsifikasyon, apendiksin anatomik seyrini gösteren seri kesitlerde apendiks lümeni ile ilişkisi doğrulanır.
  • Obstrüktif yerleşim — apendiks tabanı veya gövdesindeki kalkülün distalinde sıvı ile distansiyon ve duvar inflamasyonu bulunabilir.
  • Çevre yağ planlarının korunması — apendikolitin tek başına varlığı apandisit için güvenilir bir gösterge değildir; apendiks ve çevresindeki inflamasyon ayrıca değerlendirilir.
  • Apendiks dışına yer değiştirme — kalkül lümen sınırının dışında ve periappendiküler koleksiyon içinde bulunuyorsa ekstralüminal apendikolit düşünülür.
  • Birden fazla kalkül — lümen boyunca ayrı hiperdens odaklar izlenebilir; her birinin intralüminal ya da ekstralüminal olduğu belirlenir.
  • Eşlik eden duvar ve sıvı bulguları — apendiks çap artışı, duvar kalınlaşması, sıvı dolu lümen ve yağ kirlenmesi eşlik ederse apandisit de vardır.

Normalde

Normal apendiks lümeninde hava veya yumuşak doku dansiteli içerik bulunabilir, ancak çevresinden belirgin ayrılan fokal kalkül yoktur. Aksiyel ve koronal kesitlerde odağın apendiks lümenindeki konumunu değerlendirin; apendikolit BT’de hiperdens intralüminal kalsifikasyon olarak tanımlanır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
