Colocolic intussusception

Eğitim amaçlıdır; hasta başında tanı ve tedavi kararının yerine geçmez.

Özet

Kolokolik invajinasyonda kolonun bir segmenti, mezokolon ve damarlarıyla birlikte distal kolonik segmente teleskop gibi girer. Erişkinde BT iç içe geçmiş duvarları ve mezenterik yağı gösterirken, olası öncü lezyonu ve obstrüksiyon ya da iskemi bulgularını araştırır. Kolonik invajinasyonlarda altta yatan yapısal lezyon araştırılmalıdır; kolokolik invajinasyonda malignite olasılığı yüksektir. Mezenterik dolaşım bozulduğunda duvar konjesyonu ve parietal iskemi görülebilir; komplike invajinasyonlarda perforasyon ve peritonit riski artar.

Okuma sırası

  1. İnvajinasyon segmenti. Önce kolon lümeninde ikinci bir duvar halkası oluşup oluşmadığını bulun; iç içe geçmenin uzun eksenini çok düzlemli kesitlerde izleyin.
  2. İnvajine mezokolon. İç segmentin merkezine doğru taşınan yağ ve damar paketini takip ederek mezokolonun invajinasyona katıldığını doğrulayın.
  3. İnvajinasyon başı. İnvajinasyon başını tarayıp duvardan lümene uzanan polip ya da kitleyi olası öncü lezyon olarak arayın.
  4. Proksimal kolon ve invajine segment duvarı. Son olarak proksimal kolon genişlemesini, duvar kontrastlanmasını, serbest sıvıyı ve ekstraluminal havayı araştırın.

  • İç içe çekilmiş yağlı mezokolon ve ona eşlik eden kıvrımlı damarların lümen içine uzanması
  • İnvajinasyon başında polipoid veya asimetrik yumuşak doku kitlesi olası öncü lezyondur
  • İnvajinasyonun proksimalinde kolon çap artışı ve distalde kollaps mekanik obstrüksiyonu destekler
  • Mezenterik dolaşımın bozulmasına bağlı bağırsak duvarı konjesyonu ve parietal iskemi bulguları komplike invajinasyonu düşündürür; bu durum perforasyon ve peritonit riskinin artmasıyla ilişkilidir.

Enteroenterik: ince bağırsakla sınırlıdır; kolokolik: yalnız kolon segmentleri iç içe geçer; enterokolik: ince bağırsak kolona uzanır ve ileoçekal/ileoçekokolik alt tipler bu grupta tanımlanabilir.

Normalde

Aynı anatomik segmentte iç içe geçmiş ikinci bir duvar katmanının bulunup bulunmadığını, mezenterik yağ ve damarların lümen içine taşınıp taşınmadığını ve proksimal-distal kalibrasyon farkını kıyaslayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
