Özet

Kolorektal lenfoma nadir bir ekstranodal lenfomadır ve primer kolon tümörlerini taklit eden polipoid, infiltratif ya da uzun segmentli duvar tutulumuyla görülebilir. Kontrastlı BT, bağırsak duvarını, mezenterik ve retroperitoneal nodları, eşlik eden ince bağırsak tutulumunu ve diğer organlardaki olası hastalık bulgularını birlikte değerlendirmeyi sağlar. Adenokarsinomdan farklı olarak belirgin darlık olmadan lümen genişlemesi veya hacimli kitle oluşturabilir; kesin alt tip doku tanısıyla belirlenir. Kolorektal lenfoma diğer kolon tümörlerini taklit edebildiğinden görüntüleme bulguları yayılımı değerlendirmeye yardımcı olur, ancak kesin tanı için doku biyopsisi gerekir; perforasyon ve fistül de görülebilir.

Okuma sırası

  1. İleoçekal bileşke. Çekum ve ileoçekal valvi bulup terminal ileumu geriye doğru izleyin; yumuşak doku tutulumunun kolondan ileuma kesintisiz geçip geçmediğini saptayın.
  2. Kalınlaşmış kolon segmenti. Tutulan segmentin iç ve dış konturunu izleyin; lümende daralma mı, anevrizmatik genişleme mi bulunduğunu değerlendirin.
  3. Mezenterik lenf nodları. Mezenterik ve retroperitoneal zincirlerdeki nodların sayısını ve dağılımını izleyin; bağırsak kitlesiyle aynı yumuşak doku görünümündeki hacimli nodları işaretleyin.
  4. Ekstramural alan ve komplikasyonlar. BT'de duvar dışı gaz veya fistül şüphesini değerlendirin; ardından komşu yağ planlarını ve peritoneal yayılımı inceleyin. Histolojik alt tip BT ile belirlenemez.

Portal venöz kontrastlı abdomen-pelvis BT'de lenfomatöz duvar kalınlaşması çoğu kez homojen yumuşak doku şeklinde izlenir; mezenterik/retroperitoneal nodlar ve karın içi diğer organlar aynı fazda değerlendirilir.

BT bulguları

  • Uzun segmentli sirkumferansiyel kalınlaşma — düzgün veya hafif nodüler duvar boyunca uzanan homojen yumuşak doku infiltrasyonu görülebilir.
  • Hacimli polipoid kitle — özellikle ileoçekal bölgede lümene veya dışarıya büyüyen büyük yumuşak doku kitlesi olabilir.
  • Anevrizmatik lümen genişlemesi — lenfomada görülebilir ve kas tabakası kaybı ile otonom sinir pleksusunun yıkımına bağlanabilir; lenfomaya özgü değildir.
  • Mezenterik ve retroperitoneal adenopati — birden fazla istasyonda iri, çoğunlukla homojen nodlar eşlik edebilir.
  • Terminal ileum devamlılığı — ileoçekal valv ve terminal ileuma uzanan tutulum lenfoma olasılığını artırır.
  • Yağ planlarının korunması — hacimli kitleye rağmen komşu organ sınırları ve perikolik yağ planları korunmuş kalabilir.
  • Perforasyon veya fistül — duvar bütünlüğünün bozulması, ekstralüminal gaz ya da komşu ansa fistül şüphesi oluşturur; çevresel desmoplastik reaksiyon her zaman eşlik etmeyebilir.
  • Kolorektal lenfoma primer olabilir veya sistemik lenfomanın sekonder tutulumu şeklinde görülebilir; BT'de lokal ve uzak lenf nodları ile dalak, karaciğer ve diğer abdominal organlar da değerlendirilmelidir.

Normalde

Lenfoma şüphesinde duvar kalınlaşmasının uzunluğunu, lümenin beklenen daralma yerine genişleyip genişlemediğini ve çevre nodların normal ince mezenterik yapılardan ayrılıp ayrılmadığını kıyaslayın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Kolon adenokarsinomu
daha sık kısa, omuzlu stenoz ve proksimal dilatasyon yapar; mezokolik yağda desmoplastik çekinti veya komşu organ invazyonu olabilir.
Crohn hastalığı
kaynakta ince bağırsak için tarif edilen inflamatuvar patern, kesintili segmental sirkumferansiyel kalınlaşma ve duvar katmanlarında belirginleşme içerebilir.
İnflamatuvar hastalıkların görüntüleme bulguları kolorektal lenfoma bulgularıyla örtüşebilir; bu nedenle BT tek başına kesin ayrım sağlamayabilir.
Metastatik tümör
bilinen primer malignite, çoklu bağırsak odakları veya peritoneal implantlar metastatik tutulumu destekleyebilir.

Tuzaklar

  • Anevrizmatik genişleme lenfomayı düşündürür ama tek başına tanısal değildir; adenokarsinom, inflamasyon ve diğer kitleler de benzer lümen şeklini oluşturabilir.
