Özet

Enterokütanöz fistül, bağırsak lümeni ile deri arasında anormal bir bağlantıdır ve çoğunlukla karın ameliyatlarından sonra, daha seyrek olarak Crohn hastalığı veya tümör zemininde gelişir. Kontrastlı karın BT, bağırsak duwarındaki iç ağızdan peritoneal boşluk ve karın duwarını geçerek cilde ulaşan traktı; eşlik eden apse ve altta yatan hastalığı aynı incelemede değerlendirmeyi sağlar. Enterokütanöz fistül traktı kollabe ya da açık olabilir; sagital rekonstrüksiyon ventral karın duvarını değerlendirmede özellikle yararlıdır ve gerektiğinde fistül ağzından kontrast verilerek yapılan BT fistülografi, traktı ve bağlantılı bağırsak ansını gösterebilir. Fistülün ya da ilişkili koleksiyonun gözden kaçması sepsis kaynağını ve hastalığın yayılımını eksik göstermeye yol açabilir.

Faz ve pencere

Portal tanısal
IV kontrastlı BT'de bağırsak ansından başlayıp öne veya ön-yan tarafa yönelen, peritoneumu ve karın duvarı kasları ile fasyalarını geçerek cilde ulaşan; kollabe ya da hava ve/veya sıvı içeren tübüler trakt aranır. Trakt duvarındaki kontrastlanma ve trakt boyunca gelişebilen çevresel kontrastlanan apse de değerlendirilir. Oral kontrast, bağırsak anslarını ekstraluminal yapılardan ve koleksiyonlardan ayırmaya yardımcı olabilir; ancak ameliyat sonrası ileus veya obstrüksiyon nedeniyle bazı hastalar kontrastı içemeyebilir.

BT bulguları

  • Bir bağırsak ansından başlayıp ventral veya ventrolateral yönde peritoneal boşluğa ve karın duvarına ilerleyen tübüler ekstraluminal trakt.
  • Trakt içinde gaz ve sıvı; açık trakt boyunca eşleşen iki kontrastlı kenar görüldüğünde tram-track görünümü.
  • Traktın rektus veya oblik kasları ve fasyaları aşarak ciltte çökük, kalınlaşmış ya da açık bir fistül ağzına ulaşması.
  • Fistülografide bağırsakla ileti gösterilebilir; kontrast geçişinin görülmemesi ödem, debris veya basıya bağlı olabilir. Cilt açıklığına kadar devamlılık ayrıca izlenmelidir.
  • İç ağız komşuluğunda kalınlaşmış ve kontrastlanan bağırsak segmenti; Crohn aktivitesi, anastomoz kaçağı veya tümör kaynağını araştırmayı gerektirir.

Normalde

Normal karın duwarı katmanları arasında bağırsak lümeniyle bağlantılı tübüler bir trakt bulunmaz; patolojik durumda bu katlar transvers olarak geçilir. BT'de traktın karın duvarı kas ve fasyalarını geçerek ciltteki fistül açıklığına uzanışı değerlendirilir. Kıyasta ans duvarındaki olası iç ağızdan cilt açıklığına dek kesintisiz devamlılık, duvar kontrastlanması ve koleksiyon varlığı aranır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
