Özet

Azygos lobu, gelişim sırasında azygos veninin sağ üst lob apeksine göç ederek akciğer dokusu içinde seyrettiği nadir bir anatomik varyanttır; ayrı bir bronkopulmoner lob oluşturmaz. BT'de kıvrımlı azygos fissürü ve ucundaki azygos veni, apeksin medialinde küçük bir akciğer alanını sınırlar. Varyantı tanımak venöz yapıyı kitle veya anormal fissürü patoloji sanmayı ve girişim planında beklenmedik damar yaralanmasını önler. Azygos lobu genellikle rastlantısal saptanır ve mevcut pulmoner patoloji ile ilişkili değildir; komşu akciğer dokusundaki bulgular ayrı olarak değerlendirilmelidir.

  • Azygos fissürü — sağ üst lob apeksine doğru aşağı ve medial seyreden, düzgün ince çizgisel plevral kıvrım.
  • Fissür ucunda azygos veni — kıvrımın inferior ucunda yuvarlak veya tübüler yumuşak doku odağı.

Normalde

Azygos lobu bulunmayan normal sağ akciğer apeksinde plevral kıvrım parankim içine doğru inmez; azygos veni sağ trakeobronşiyal açı düzeyindeki arkusuna normal mediastinal güzergâhından ulaşır. Sağ üst lob apeksindeki kavisli plevral çizginin yönünü, inferior ucundaki venöz yapıyı ve fissürün çevrelediği alanda korunmuş akciğer parankimini izleyin.
