Özet

Primer nonseminomatöz mediastinal germ hücreli tümör, sıklıkla genç erişkin erkeklerde ortaya çıkan ve hızlı değerlendirme gerektiren malign ön mediastinal kitledir. Kontrastlı BT, kitlenin solid ve nekrotik bileşenlerini, damar/organ ilişkisini ve akciğer ya da plevra yayılımını haritalar; tümör belirteçleri ve histolojiyle birlikte yorumlanmalıdır. Büyük tümörler heterojen olabilir, komşu yapılara uzanabilir ve akciğer nodülleriyle eşlik edebilir. Santral düşük atenüasyonlu alanlar nekroz, hemoraji veya kistik dejenerasyonu yansıtabilir; ayrıca mediastinal germ hücreli tümörlerde nadiren damar içi tümör trombüsü görülebilir. Değerlendirme serum AFP ve β-hCG belirteçleri ile birlikte yapılır; primer mediastinal odak düşünülürken gonadal primer de klinik olarak dışlanmalıdır.

Faz ve pencere

Kontrastlı tanısal
Ön mediastende büyük, sıklıkla heterojen yumuşak doku kitlesi; santral düşük atenüasyonlu nekroz/kistik alanlar ile kontrastlanan periferik solid dokunun ayrımı ve komşu damar, perikard, plevra ile akciğer ilişkisinin incelenmesi için kullanılır.

Önerilen pencereler: Mediasten (G 350 / M 50), Yumuşak doku (G 400 / M 40), Akciğer (G 1500 / M -600).

BT bulguları

  • Prevasküler ön mediastende geniş, lobüle veya düzensiz sınırlı solid kitle.
  • Heterojen atenüasyon; santral düşük yoğunluklu nekroz, kistik değişiklik veya hemoraji alanları.
  • Canlı periferik yumuşak doku bileşeni kontrastlanabilir ve nekrotik merkezden ayrılır.
  • Kitle akciğer, plevra, perikard veya göğüs duvarıyla yağ planını silebilir; doğrudan invazyon ve bası ayrı tanımlanır.
  • Aort, ana pulmoner arterler ve vena kava ile temas, sarılma, lümen daralması veya tromboz açısından incelenir.
  • Akciğer parankimindeki ayrı nodül veya kitleler olası metastatik yayılım açısından primer odak ve klinik bağlamla birlikte değerlendirilir.
  • Plevral ya da perikardiyal efüzyon, atelektazi ve hava yolu basısı ileri yerel hastalıkla birlikte görülebilir.
  • Nonseminomatöz mediastinal germ hücreli tümörler embriyonel karsinom, koryokarsinom, yolk sac tümörü, mikst germ hücreli tümörler ve malign transformasyonlu teratomu kapsar; görüntüleme histolojik tanıyı tek başına kesinleştiremez ve biyopsi gerekebilir.

Normalde

Normal prevasküler mediastende timus kalıntısı ve yağlı planlar damarlarla perikard arasında korunur; büyük, heterojen, nekrotik yumuşak doku kitlesi bulunmaz. Kitlenin aort yayı, ana pulmoner arterler, vena kava superior, trakea ve perikardla temasını normal kontur ve lümenlerle kıyasla; akciğer penceresinde ayrı nodülleri kitleye komşu atelektaziden ayır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Mediastinal seminom
daha çok homojen solid kitle görünümü verir; geniş nekroz ve düzensiz invaziv kenar daha az tipiktir.
Primer mediastinal büyük B hücreli lenfoma da ön mediastende homojen veya düşük atenüasyonlu nekrotik/kistik alanlar içeren yumuşak doku kitlesi şeklinde görülebilir ve görüntülemede germ hücreli tümörden ayırt edilmesi güç olabilir. Lenf nodu dağılımı, klinik bulgular ve doku tanısı ayırıcı değerlendirmeye katkı sağlar.
Matür teratom
yağ, kistik sıvı ve kalsifikasyon/kemik bileşenlerinin birlikteliği ayrım sağlar.
Timik karsinom
düzensiz invaziv ön mediastinal kitle olabilir; yaş ve klinik bağlam ile doku örneklemesi ayırıcıdır.
Timoma
çoğunlukla daha sınırlı timus yatağı kitlesidir; plevral implant veya komşu invazyon ileri hastalıkta görülebilir.

Tuzaklar

  • Santral düşük atenüasyonlu alanlar yalnız nekroz değil, kistik dejenerasyon veya proteinaceous sıvı içeriği de olabilir; kontrast sonrası periferik duvar/komponentlerin tutulumu ve MRI'da T2 hiperintensitesi ile solid-kistik ayrımı desteklenmelidir.
  • Büyük damarların kitleye yakın seyri invazyonu kanıtlamaz; lümen devamlılığı, daralma, duvar düzensizliği ve kollateral dolaşımı ayrı ayrı değerlendir.
  • Tümör belirteçleri veya histoloji olmadan BT görünümünden germ hücreli tümör alt tipi kesinleştirilmemelidir; rapor morfoloji ve yayılımı tanımlar.
  • Plevral sıvı ya da komşu konsolidasyon tek başına metastaz değildir; doğrudan uzanım, bası atelektazisi ve eşlik eden enfeksiyon olasılığını ayır.
