Mediastinal seminom

Özet

Primer mediastinal seminom, çoğunlukla genç erişkin erkeklerde görülen malign ekstragonadal germ hücreli tümördür. BT kitleyi lokalize edip uzanımını belirlemeye yardımcı olur; seminomlar tipik olarak solid, homojen yoğunlukta ve hafif kontrastlanan kitlelerdir, ancak ön mediastinal tümörlerin görüntüleme özellikleri örtüşebilir ve seminomda akciğer ya da bölgesel nod metastazları görülebilir. Kitle komşu yapıları itebilir; klasik seminomda doğrudan invazyon tipik değildir ve nadirdir. Görüntüleme histolojik alt tipi tek başına kanıtlamaz, ancak yayılımı atlamak hastalık yükünün eksik raporlanmasına neden olabilir.

Kontrastlı tanısal

Seminom BT'de genellikle solid, homojen yoğunlukta ve hafif kontrastlanan bir kitle olarak görülür; kistik dejenerasyon bulunabilir ve komşu yapı invazyonu nadirdir.

Önerilen pencereler: Mediasten (G 350 / M 50), Yumuşak doku (G 400 / M 40), Akciğer (G 1500 / M -600).

BT bulguları

• Seminomlar ön mediastende yerleşen, BT'de büyük ve solid kitleler olarak görülebilir.
• Yumuşak doku atenüasyonu tipik olarak görece homojendir ve hafif kontrastlanır; bazı olgularda kistik dejenerasyon izlenebilir.
• Seminomda komşu yapılara invazyon nadirdir; kitlenin mediastinal yapılarla temas kapsamı ve aradaki yağ planı invazyon değerlendirmesinde dikkate alınır.
• Kitle ile komşu damarlar arasındaki temas düzeyi ve ara yağ planının kaybı invazyon açısından değerlendirilir; ancak bu bulguların tanısal hassasiyeti değişkendir.
• Trakea, ana bronş, kalp veya perikard üzerinde bası ve yer değiştirme görülebilir.
• Akciğer nodülü, plevral odak veya ek mediastinal/hiler nodal büyüme yayılım açısından taranır.
• Büyük mediastinal seminom kitlesi SVC'ye bası yapabilir; nadir olgu bildiriminde SVC invazyonu da tanımlanmıştır.
• Seminomda vena kava superior invazyonu bildirilmiştir; kitle ile komşu mediastinal yapılar arasındaki temas kapsamı ve ara yağ planlarının silinmesi invazyon değerlendirmesinde kullanılır, ancak invazyonu güvenilir biçimde öngörmez.

Normalde

Timus, 20 yaş altındaki kişilerde ağırlıklı olarak lenfoid dokudan oluşur; daha sonra küçülür ve yağlı involüsyona uğrar. Kitleyi damarlarla temas ettiği yerde değerlendir; kitle-damar temas kapsamını ve aradaki yağ planlarının silinip silinmediğini ayrı ayrı belirt.

Ayırıcı tanı

Primer mediastinal büyük B hücreli lenfoma

düzenli konturlu kitle, damar tutulumu ve servikal/abdominal lenfadenopatının genellikle yokluğu ile karakterizedir.

Nonseminomatöz germ hücreli tümörler BT'de heterojen, nekroz, kanama veya kistik dejenerasyona bağlı hipodens alanlar içeren iri ve düzensiz kitleler olarak görülebilir; komşu yapıları invaze edebilir.

Timik epitel tümörleri BT'de genellikle homojen yumuşak doku kitlesidir; düşük riskli timomalar düzgün konturlu ve tam ya da tama yakın kapsüllü, yüksek riskli timomalar ise lobüle konturlu ve mediastinal yağ veya büyük damar invazyonu gösterebilir.

Matür teratom

yağ, sıvı ve kalsifikasyon gibi farklı atenüasyonları bir arada içerir.

Substernal guatr

tiroidle devamlılık, servikal uzanım ve iyotlu kontrastta belirgin boyanma gösterebilir.

Tuzaklar

• Damarı saran kitleyi otomatik olarak damar invazyonu sayma; ince kesitlerde damar lümenini ve duvarını tüm temas boyunca izle.
