Özet

Özofagus dismotilitesi primer olarak peristaltizmin bozulması ve alt özofageal sfinkter gevşemesinin yetersizliği ile karakterizedir; BT'de izlenen tortiyöz seyir ve lümen retansiyonu yapısal eşlikçilerdir ancak kesin tanı ve alt tip sınıflaması için yüksek çözünürlüklü manometri ve baryumlu inceleme gereklidir. BT anatomik genişleme, divertikül, dıştan bası ve aspirasyon gibi eşlik eden durumları gösterir, ancak geçici kasılmayı ya da motilite alt tipini güvenilir biçimde tanımlamaz. Spazmın klasik corkscrew/rosary-bead görünümü dinamik baryumlu özofagogramda değerlendirilir; kesin fizyolojik sınıflama manometriyle yapılır. Düzensiz sabit darlığı geçici kasılma sanmak altta yatan tümör veya striktürü geciktirebilir.

Özofagus kalibrasyonu, seyri, lümende hava-sıvı retansiyonu ve akciğerlerde aspirasyon bulguları görülebilir; bu tek zamanlı anatomik görüntü geçici spazmı veya peristaltizmi tanılamaz.

Önerilen pencereler: Mediasten (G 350 / M 50), Akciğer (G 1500 / M -600), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Özofagus ekseninde uzama, kıvrımlanma veya toraks içinde yer değiştirme; gerçek tortiyöz seyri ardışık kesitlerde takip et.
  • Özofagus lümeninde köpüklü sıvı, hava-sıvı içeriği veya yutulmuş materyal retansiyonu.
  • BT’de distal özofagus daralması görülebilir; dar segmentin düzgün ya da düzensiz morfolojisi primer akalazya ile psödoakalazya ayrımına katkı sağlar. Motilite bozukluğu dinamik inceleme ve yüksek çözünürlüklü manometri ile değerlendirilir.
  • Psödoakalazya durumunda distal özofagus duvar kalınlaşması nodüler veya lobüle ve asimetrik olabilir. Klinik şüphede endoskopi ve gerekirse endosonografi değerlendirilir.
  • Epifrenik divertikül veya hiatal herni gibi stazla ilişkili anatomik eşlikçiler.
  • Retansiyon ve regürjitasyon zemininde alt loblarda aspirasyonla uyumlu küçük hava yolu opasiteleri.
  • Distal özofagus spazmı, BT görünümüyle değil yüksek çözünürlüklü manometride normal özofagogastrik bileşke gevşemesi eşliğinde en az %20 erken kasılma bulunmasıyla tanımlanır; klinik olarak anlamlı tanı için disfaji veya kardiyak olmayan göğüs ağrısı da gerekir.

Normalde

Normal özofagus lümeni kollabe durumdadır; salya veya yiyecek retansiyonu ile belirgin dilatasyon veya tortüozluk görülmez. Tek zamanlı BT görüntüsündeki geçici lümen daralması peristaltik bozukluk kanıtı değildir; dinamik değerlendirme için manometri veya baryumlu inceleme gereklidir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Tuzaklar

  • BT'de görülen özofageal daralma statiktir; spazm tanısı için dinamik manometri veya baryumlu inceleme gereklidir ve malignite dahil yapısal nedenler öncelikle dışlanmalıdır.
  • Tortiyöz özofagus görünümü tek başına motilite bozukluğunu doğrulamaz; lümen retansiyonu ve diğer bulgularla birlikte değerlendirilip tanı manometriyle doğrulanır.
  • Dilate özofagus lümenindeki salya veya gıda retansiyonu, yumuşak doku lezyonuyla karışabilir; bu nedenle lümen içeriği ile duvar arasındaki ilişki dikkatle ayrıştırılmalıdır.
