Renal hücreli karsinom (RCC), erişkinlerde böbrek parankiminden kaynaklanan başlıca malign solid tümördür ve görünümü alt tipe göre değişir. Kontrastlı çok fazlı BT, kitleyi saptama/karakterize etmenin yanında renal sinüs, perinefrik yağ, renal ven ve vena kava yayılımını, karşı böbreği ve abdominal metastaz şüphesini aynı incelemede değerlendirir. Venöz tümör trombüsünün kraniyal uzanımını veya komşu doku invazyonunu atlamak cerrahi evrelemeyi ve tedavi planını etkileyebilir; perinefrik yağ çizgilenmesi tek başına invazyon kanıtı sayılmamalıdır.
Faz ve pencere
- Kontrastsız tanısal
- Kitle atenüasyonunun bazali belirlenir; kanama, yağ ve kalsifikasyon araştırılır. Bu seri postkontrast HU değişimini ölçmek için referanstır ve bazı papiller RCC'ler kontrastsız görüntüde hiperdens olabilir.
- Kortikomedüller tanısal

BT bulguları
- Renal korteks veya medulladan gelişen, normal parankimden farklı kontrastlanan solid kitle; berrak hücreli tipte erken güçlü ve sıklıkla heterojen kontrastlanma görülebilir.
- Nekroz, kistik dejenerasyon, kanama veya kalsifikasyon nedeniyle kitle içi atenüasyon ve kontrastlanma heterojen olabilir.
- Renal sinüs yağının silinmesi, kaliks/pelvis basısı veya invazyonu ve renal parankim dışına uzanan yumuşak doku lokal yayılım kuşkusunu artırır.
- Renal ven ya da VCI içinde devamlı dolum defekti; kitleyle doğrudan bağlantı ve trombüs içinde kontrastlanma tümör trombüsü lehinedir.
- Tümör trombüsünün renal ven boyunca ilerleyip VCI'ye uzanımı koronal/sagital reformatlarda kraniyal uç düzeyiyle birlikte gösterilir.
- Perinefrik yağ infiltrasyonunun lineer-nodüler veya nodüler görünümü, renal hücreli karsinomda perinefrik yağ invazyonunu öngören bulgulardandır.
- Karşı böbrekte eşzamanlı solid kitle, böbrek fonksiyonel rezervi ve cerrahi seçeneklerin değerlendirilmesinde ayrıca belirtilir.
- Adrenal bez, karaciğer, lenf nodları ve görüntü alanındaki kemiklerde metastatik hastalık düşündüren odaklar aranır.
- RCC evrelemesinde toraks BT akciğer metastazlarını gösterebilir; ancak klinik T1 hastalarda rutin preoperatif toraks BT’nin tanısal verimi düşük olabilir.
Ölçütler ve sınıflamalar
- Renal kitlede kesin kontrastlanma eşiği (attenüasyon artışı)
- Çalışmalarda solid renal kitle için >15 HU veya >20 HU kontrastlanma eşikleri kullanılmıştır; ≥20 HU bu kaynakta evrensel kesin eşik olarak doğrulanmamaktadır.
- Kontrastlanmama ve şüpheli aralık
- Solid renal kitlelerde kontrastlanma için çalışmalarda >15 HU veya >20 HU eşikleri kullanılmıştır.
- RCC'de ekstrarenal yağ ve renal ven invazyonu (T3a)
- T3a evresi için renal ven tutulumu (ana ven veya segmental dal) ya da perirenal veya renal sinüs yağ invazyonu temel bulgulardandır. T3c hastalık, hepatik ven çevresindeki trombüs veya VCI duvarı invazyonu şüphesini içerebilir.
Normalde
Renal korteks medulladan daha vaskülerdir; kortikomedüller faz, erken kontrastlanma sırasındaki bu perfüzyon farkını değerlendirmek için uygun penceredir. Karşılaştırmada kitleyi aynı böbreğin komşu parankimi ve karşı böbreğin aynı fazdaki dokusuyla kıyaslayın; renal ven ve VCI lümenlerinin kesintisiz kontrast dolumunu, renal sinüs yağını ve perinefrik planları izleyin.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Renal kitle çevresinde yalnız ince perinefrik yağ çizgilenmesi var; ven lümenleri açık ve kitle dışına nodüler uzanım yok. Hangisi en doğru yaklaşımdır?
