Özet

Çift toplayıcı sistemde aynı böbreğin üst ve alt parankim bölümleri ayrı pelvikalisiyel ünitelerle drene olur; üreterler mesaneye ulaşmadan birleşebilir veya ayrı ağızlarla sonlanabilir. BT ürografinin ekskretuar görüntüleri, opaklaşan iki kaliks-pelvis grubunu ve üreterlerin birleşme ya da ayrı seyir biçimini gösterir. Çoğu olgu tesadüfi olsa da üst pol üreterinde ektopik sonlanma veya üreterosel ile tıkanma, alt polde ise reflüye bağlı sorunlar eşlik edebilir. Bir üniteye ait genişlemeyi diğerinin normal görünümü içinde gözden kaçırmak, segmental obstrüksiyonun yerini yanlış belirlemeye yol açar.

Faz ve pencere

BT ürografi tanısal: Ekskretuar fazda opaklaşmış iki ayrı kaliks-pelvis grubu ile üreterlerin mesaneye kadar ayrı devam edip etmediği ya da birleşerek tek distal üreter oluşturduğu değerlendirilir. Opaklaşma yetersiz veya gecikmişse bir ünite görünmeyebilir; bu durumda tek görüntüden tam ya da inkomplet duplikasyon sonucuna varılmaz.

Önerilen pencereler: Batın (G 400 / M 50).

BT bulguları

Aynı böbrek konturu içinde üst ve alt polü drene eden iki ayrı kaliks-pelvis ünitesi; koronal MPR'de her birinin ayrı drenajını gösterme.
Distal üreterlerin sonlanımı BT'de gösterilemiyorsa yalnız izlenebilen seyri tarif edin; bir olguda nonkontrast BT distal sonlanmayı belirleyememiştir.
İnkomplet duplikasyonda iki proksimal üreterin daha aşağıda birleşip tek üreter halinde devam etmesi; birleşme noktası görüntülenebiliyorsa belirtme.
Üst pol parankiminde küçülme, incelme veya skar; ilgili üniteyi drene eden sistemin opaklaşma ve genişleme durumuyla birlikte değerlendirme.
Üst pol toplayıcı sisteminde kaliks genişlemesi ve dilate üreter; eşlik eden distal üreterosel veya ektopik üreter sonlanmasını arama.
Alt pol ünitesinde kaliks ya da pelvis genişlemesi ve üreter seyri boyunca olası darlık, taş veya duvar kalınlaşması.
Her iki ünitede taş veya hidronefroz varlığı araştırılır; bulgunun hangi drenaj ünitesine ait olduğunun belirlenmesi önemlidir.
Komplet duplikasyonda Weigert–Meyer ilişkisine göre üst pol üreteri genellikle obstrüksiyonla, alt pol üreteri vezikoüreteral reflüyle ilişkilidir; bu örüntünün istisnaları olabilir.
Komplet duplikasyonda distal üreter bağlantısı BT'de gösterilemeyebilir; bir olguda nonkontrast BT distal sonlanmayı belirleyememiş, MR ürografi komplet sistemi ortaya koymuştur.

Ölçütler ve sınıflamalar

Üreter duplikasyonunun anatomik tipi: İnkomplet duplikasyonda bifid üreterler, komplet duplikasyonda ise iki ayrı üreter görülebilir.

Normalde

Normal böbrekte tek pelvikalisiyel sistem renal pelviste birleşir ve tek üreter olarak mesaneye iner; her iki böbreğin konturu ve parankim kalınlığı karşılaştırılabilir. Duplikasyonda aynı böbrek içindeki üst ve alt kaliks gruplarının ayrı pelvis oluşturup oluşturmadığını, opaklaşan üreterlerin birleşme seviyesini ve her ünitedeki parankim bütünlüğünü karşılaştırın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
