Özet

Renal onkositom, erişkinlerde görülen benign epitel kökenli bir böbrek tümörüdür ve çoğu kez tesadüfen saptanan solid kortikal kitle olarak karşımıza çıkar. BT, kitlenin böbrek içindeki konumunu, kontrastlanmasını ve çevre anatomik yapılarla ilişkisini gösterir; ancak onkositomu özellikle kromofob renal hücreli karsinomdan güvenilir biçimde ayıramaz. Santral yıldızsı skar ve segmental kontrastlanma farklılığı tanıyı tek başına kanıtlamaz. Görüntüyü kesin benign kabul etmek eşlik eden veya taklit eden malign tümörün gözden kaçmasına yol açabilir.

Faz ve pencere

Kontrastsız
Solid kitle böbrek korteksinden kaynaklanan yumuşak doku dansitesinde görülür; kontrastsız atenüasyon tek başına onkositomu renal hücreli karsinomdan ayırmaz. Yağ içeriği veya kalsifikasyon varsa ayrıca kaydedilir.
Kortikomedüller
Kortikomedüller fazda onkositom belirgin ve bazen heterojen kontrastlanabilir; tümör içindeki santral düşük atenüasyonlu skar benzeri bölüm çevre solid dokudan daha az kontrastlanabilir. Bu görünüm berrak hücreli veya kromofob karsinomla örtüşür.
Nefrografik
Nefrografik fazda kitle ile normal korteks arasındaki kontrast farkı ve olası heterojen bileşenler izlenir; bazı olgularda tümörün bir bölümü diğerine göre daha erken kontrastlanmış görünür (segmental kontrastlanma inversiyonu). Bulgular tanısal değildir.
Gecikmiş
Gecikmiş görüntülerde daha önce düşük atenüasyonlu olan santral stromal alan kontrastlanabilir ve kitle daha homojen hale gelebilir; bu örüntü tüm olgularda görülmez ve tek başına benignlik göstermez.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Korteksten dışa doğru büyüyebilen, yuvarlak-oval, iyi sınırlı solid renal kitle; ekzofitik veya endofitik bileşenini belirt.
  • Belirgin, homojen ya da heterojen kontrastlanma; kontrastlanma derecesi faza göre değişebilir.
  • Santral yıldızsı skar benzeri düşük atenüasyonlu alan, özellikle büyük kitlelerde görülebilir ancak kromofob renal hücreli karsinomda da bulunabilir.
  • Segmental kontrastlanma inversiyonu, kontrastlı iki fazlı BT'de değerlendirilen bir bulgudur; bildirilen duyarlılığı çalışmalar arasında belirgin değişkenlik gösterir.
  • Onkositom BT'de heterojen solid kitle olarak görülebilir; görüntüleme bulguları tek başına histolojik tanı koydurmaz.
  • Renal sinüs, kaliksler, renal ven ve perirenal yağla temas ya da invazyon şüphesini ayrı ayrı değerlendir.
  • Eşlik eden ikinci renal kitle, bölgesel lenf nodu veya uzak metastaz şüphesi varsa primer solid kitleden bağımsız tarif et.
  • Kontrastlanmayı değerlendirin; kontrastlanan renal kitlelerin çoğu görüntüleme ile benign veya malign olarak sınıflandırılamaz.

Normalde

Karşılaştırmada kitlenin böbrekteki yerini BT'de tanımlayın; santral yıldızsı skar onkositoma özgü değildir. Kontrastlı BT'de kitle atenüasyonunu renal korteksle oranlayın; kontrastlanma ve heterojenliği kortikomedüller, nefrografik ve boşaltım fazlarında değerlendirin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Kromofob renal hücreli karsinom
iyi sınırlı ve homojen kontrastlanan kitle, santral skar ve segmental kontrastlanma farklılığı onkositomla örtüşebilir; BT güvenilir histolojik ayrım sağlamaz.
Berrak hücreli renal hücreli karsinom
sıklıkla belirgin arteriyel kontrastlanma gösteren solid kortikal kitle; nekroz ve heterojenlik büyüdükçe belirginleşebilir.
Papiller renal hücreli karsinom
çoğunlukla diğer solid renal tümörlere göre daha az kontrastlanır ve nefrografik fazda hipodens kalabilir.
Yağdan fakir anjiyomiyolipom
kortikal solid kitle olarak görünür; makroskopik yağ yoksa BT ile renal hücreli karsinomdan ayırmak güçleşir.
Basit veya kompleks renal kist
sıvı atenüasyonu ve kontrastlanan solid nodül bulunmaması kistik lezyonu solid onkositomdan ayırır.

Tuzaklar

  • Santral yıldızsı skarı onkositom için patognomonik saymayın; kromofob ve berrak hücreli karsinomlarda da skar ya da nekroz görülebilir.
  • Segmental kontrastlanma inversiyonu tek başına onkositom tanısı koydurmaz; faz zamanlaması ve tümör heterojenliği görünümü değiştirebilir.
  • İyi sınırlı kontur benign histoloji kanıtı değildir; renal sinüs, ven ve perirenal yağdaki invazyon bulgularını ayrıca arayın.
