Özet

Hidrosel tunika vaginalis yaprakları arasında sıvı, hematosel ise bu boşlukta kan birikmesidir; travma, cerrahi veya başka bir skrotal süreç sonrasında görülebilir. Skrotum BT'de çoğu kez tesadüfen değerlendirilir; basit sıvıyı gösterebilir ve travma bağlamında yüksek dansiteli ya da tabakalanan kan ürünlerini düşündürebilir, ancak skrotal ultrason ve Doppler testis dokusu ile perfüzyon için daha uygundur. Hematosel testisi sıkıştırabilir ve testis yırtılmasıyla birlikte bulunabilir. Büyük kanama, rüptür veya eşlik eden yaralanmanın gözden kaçması testis canlılığını tehdit edebilir.

Kontrastsız tanısal
BT incelemelerinde tunika vaginalis çevresinde kan birikimi veya basit sıvı koleksiyonları izlenebilir; ancak kesin içerik tanısı için ultrasonografi ve patolojik değerlendirme önceliklidir.
Portal
Skrotal travma değerlendirmesinde ilk tercih ultrasonografi olup; testis perfüzyonu ve kan akımının değerlendirilmesi için Doppler ultrason gereklidir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Batın (G 400 / M 50).

BT bulguları

  • Testisin çevresinde tunika vaginalis yaprakları arasında, skrotal kese konturunu izleyen sıvı koleksiyonu.
  • Basit hidroselde homojen düşük atenüasyonlu sıvı; eşlik eden inguinal kanal boyunca uzanım varsa patent processus vaginalis veya ilişkili herni olasılığı.
  • Hematoselde ultrasonografide basit hidroselden farklı olarak bağımlı ekojen debris ve daha belirgin retiküler septasyonlar görülebilir.
  • Koleksiyonun testisi çevreleyerek sıkıştırması ve skrotumda hacim artışı; parankim ayrıntısı BT'de sınırlı olabilir.
  • Skrotal duvar kalınlaşması ve çevre yağ dokusunda çizgilenme; travma, enfeksiyon veya işlem sonrası değişiklikler bağlamında değerlendirilir.
  • Hematosel ile birlikte testis kontur düzensizliği ya da tunika albuginea devamlılığında kuşku; rüptür için hedefli ultrason gerekir.
  • Pelvik sıvıyla patent inguinal kanal boyunca skrotuma inen devamlı koleksiyon; travma veya girişim sonrası bağlantılı kan/sıvı birikimini düşündürür.

Normalde

Karşı hemiskrotumla testis çevresi, tunika vaginalis boşluğu ve inguinal kanalın alt ucu kıyaslanarak sıvının tek taraflı olup olmadığına ve içeriğin basit mi kanamalı mı göründüğüne bakın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Pyocele
kalın duvarlı veya septalı kompleks koleksiyon, duvar kontrastlanması ve çevre inflamatuvar değişikliklerle enfeksiyonu destekler; BT tek başına içeriğin pürülan olduğunu kanıtlamaz.
Skrotal hematom
kanama skrotal duvar ya da yumuşak dokuda merkezlenir; hematosel tunika vaginalis boşluğunda testisi çevreler.
İnguinoskrotal herni
kese içinde yağ veya bağırsak ansı ile kanal boyunca devamlılık bulunabilir; yalnız sıvı içeren hidroselden farklı olarak lümen gazı ve bağırsak duvarı görülür.
Epididim veya funikül kökenli kistik lezyonlar genellikle tunika vaginalis ile iletişim kurmayan, testise komşu veya ayrı lokalize kistik odaklar olarak görünür.
Testis içi tümör veya hematom
odak testis parankimi içinde yer alır; BT'de ayırım kısıtlı olduğundan ultrasonla intratestiküler yerleşim doğrulanmalıdır.

Tuzaklar

  • Skrotal kistik lezyonlarda ultrasonografide anekoik görünüm, kompleks mimariyi veya ince septaları maskeleyebilir; bu nedenle basit hidrosel tanısı yanıltıcı olabilir.
  • Testis çevresindeki hematosel, küçük parankim yırtığını örtebilir; belirgin koleksiyon görülmesi tunika albuginea bütünlüğünün korunduğunu göstermez.
  • Kontrastlı fazda skrotal duvarın reaktif kontrastlanmasını testis perfüzyonu sanmayın; testis kan akımı için Doppler ultrason kullanılır.
  • Skrotuma inen pelvik sıvı, bağımsız skrotal hidrosel gibi görünebilir; koronal planda koleksiyonu inguinal kanal boyunca üst pelvis yönünde takip edin.
