Özet

Testis germ hücreli tümörleri 20–44 yaş erkeklerde en sık görülen solid tümörlerdir; metastatik testis kanserinde yayılım önce retroperitoneuma, ardından akciğerlere veya diğer visseral organlara olur. Skrotal kitleyi saptamak ve karakterize etmek için ilk inceleme ultrasonografidir; BT’nin ana rolü primer testis dokusunu değerlendirmek değil, evreleme ve yayılım haritasını çıkarmaktır. Karın-pelvis kontrastlı BT’de lateralisiteye uygun paraaortik/interaortokaval nodlar, nodal konglomeralar ve visseral metastazlar aranır. Boyut ölçütlerine dayalı BT, erken lenf nodu metastazlarını güvenilir biçimde saptamayabilir; retroperitoneal hastalığın gözden kaçması evrenin eksik belirlenmesine yol açabilir.

Portal tanısal
Evreleme için toraks-abdomen-pelvis BT kullanılır; bu incelemede bölgesel lenf nodları ve uzak metastaz odakları değerlendirilir. Skrotum görüntü alanına girerse kaba asimetri ya da kitle görülebilir ancak bu, primer tümörün karakterizasyonu değildir.

BT bulguları

  • Karın-pelvis BT’de retroperitoneal nodal hastalık, özellikle testis tarafına uygun paraaortik veya interaortokaval zincirde başlar
  • Testis primerinin tarafına göre paraaortik (sol) veya interaortokaval (sağ) zincir, seminoma metastazlarının sık görüldüğü retroperitoneal bölgelerdir.
  • Retroperitoneal nodal hastalıkta nekroz görülebilir; ancak nodun BT görünümü histolojik içeriğini (nekroz-fibrozis veya canlı tümör/teratom) güvenle belirlemez.
  • Sol testis primerinde sol paraaortik; sağ testis primerinde interaortokaval/parakaval nodal zincirlere özellikle dikkat edilir
  • Diyafram üstü lenf nodu, akciğer nodülü veya başka visseral odaklar uzak yayılım olasılığını gösterir
  • Skrotum taramaya girdiyse BT'de testis boyut asimetrisi veya kaba solid intratestiküler kitle görülebilir; ancak BT primerin karakterizasyonu için değil evreleme için kullanılır.
  • Beklenen lenfatik drenaj sol primerde paraaortik, sağ primerde interaortokaval zincirlerdir; bununla birlikte karşı taraf zincirlerinde de mikrometastaz saptanabilir.

Ölçütler ve sınıflamalar

AJCC 8. baskı klinik N kategorisi
BT’de lenf nodu boyutu değerlendirilir; ancak tek başına nod hacmi histopatolojik durumu güvenilir biçimde belirlemez. Evreleme değerlendirmesinde AFP, total hCG ve LDH serum belirteçleri de dikkate alınır.
Yayılım düzeyine göre M kategorisi

Normalde

Evreleme BT’sinde küçük lenf nodlarının boyuta dayalı değerlendirilmesi erken metastazı güvenilir biçimde dışlayamaz. Sağ ve sol gonadın beklenen lenfatik drenaj alanlarını ayrı ayrı tarayıp en büyük nodal kitlenin en uzun boyutunu ölç; CT boyutu patolojik N kategorisini ve tüm evreyi tek başına belirlemez.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Lenfoma testis kitlesinin ayırıcı tanısında düşünülmelidir; beta-hCG ve diğer bulgular tek başına değil, klinik bağlamla birlikte değerlendirilmelidir.

kitle şühesiyle değerlendirilen hastada vazit gibi inflamatuvar süreçler saptanabilir ve ileri değerlendirme gerektirir.
Skrotal herni
testis/paratestiküler kitleyi taklit edebileceğinden ayırıcı tanıda akılda tutulmalıdır.

Tuzaklar

  • Retroperitoneal lenf nodları küçük olsa da erken metastaz bulunabilir; BT’nin boyut ölçütleri metastazı güvenilir biçimde dışlayamaz.
  • Nodal eşiklerde ölçüm ekseni ve cN/pN ayrımı önemlidir; BT’de en büyük nodal kitleyi ölçmek patolojik nod sayısını belirlemez.
