Özet

Retroperitoneal kompartıman anatomisi, kanama ya da kitle odağının kaynağını ve fasya planları boyunca yayılımını anlamaya yardımcı olur. Klasik BT şeması perirenal, anterior pararenal ve posterior pararenal alanları tarif eder; güncel anatomi yaklaşımı bunları tamamen kapalı boşluklar yerine süreklilik gösteren ekstraperitoneal yağ ve fasya planlarının yaklaşık haritası olarak ele alır. Renal fasya yaprakları ve komşu organ ilişkileri ince kesit BT’de seçilebilir, ancak her hastada kesintisiz çizgi gibi görülmez. Bu planlar arasındaki bağlantıyı göz ardı etmek, hematom veya sıvının yayılımını yanlış sınırlayabilir.

Okuma sırası

  1. Aorta, vena kava ve renal hiluslar. Renal hilus düzeyindeki aksiyel kesitte önce aortayı, vena kavayı ve iki böbreği bulun; bu sabit yapıların çevresindeki yağ aralıklarını gözle izleyin.
  2. Perirenal kompartıman. Böbreğin çevresindeki yağlı zarfı takip edip anterior ve posterior sınırlarını seçin; keskin bir çizgi aramak yerine fasya ile yağ arasındaki geçişe odaklanın.

  4. Renal fasya, psoas ve periaortik yağ. Komşu kesitlerde renal fasya yapraklarının uzanımını kontrol edin; psoasların simetrisini ve periaortik dokunun yağ planını koruyup korumadığını birlikte okuyun.

Normalde

Klasik şemada retroperitoneum perirenal, anterior pararenal ve posterior pararenal kompartımanlara ayrılır; sınırlar posterior parietal periton, transversalis fasya ve perirenal fasya ile tarif edilir. Güncel yaklaşımda renal fasya ve füzyon fasyalarının oluşturduğu planları, retroperitoneal hastalık yayılımına aracılık edebilecek yollar olarak dikkate alın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
