Retroperitoneal liposarkom

Özet

Retroperitoneal liposarkom, erişkinde retroperitoneal yağ dokusundan gelişebilen ve büyük hacme ulaşana kadar sessiz kalabilen malign mezenkimal tümördür. BT, makroskopik yağı, yumuşak doku bileşenlerini ve kitlenin böbrek, kolon, psoas ile büyük damarlarla ilişkisini birlikte ortaya koyar. Yağ içeren her kitle liposarkom değildir; görüntü, organ kökeni ve iç mimari tanısal olasılığı şekillendirir, kesin alt tip için patolojik inceleme gerekir. Yağlı tümör uzanımının sınırını veya eşlik eden solid odağı atlamak hastalığın boyutunu ve olası dediferansiyasyonu eksik göstermeye yol açabilir.

Okuma sırası

1. Renal hilus ve perirenal yağ. Renal hilus düzeyinde önce böbrek dışındaki retroperitoneal yağ planına bakın; kitleyi renal parankimden ayrı başlatan bölümü gösterin.
2. Tümör içi septalar. Yağlı kitlenin içinden geçen fibröz bantları takip edin; düzgün ince çizgilerle düzensiz veya belirgin kalın septaları karşılaştırın.
3. Yağ dışı nodül. Yağ atenüasyonundan farklı nodüler dokuyu bulun ve portal fazda kontrastlanmasının çevre yağlı matriksten nasıl ayrıldığını gösterin.
4. Komşu organlar ve kaudal tümör sınırı. Kapanışta böbrek, kolon ve renal damarların yön değişimini izleyip psoas boyunca ya da pelvise uzanan ek tümör dokusu olup olmadığını tarayın.

Faz ve pencere

Kontrastsız tanısal

Subkutan yağla benzer düşük atenüasyonlu tümör bölümleri, içlerindeki yumuşak doku septaları ve nodüllerinden ayrılır; nadir de olsa kalsifik odaklar kontrastlanma olmadan izlenebilir.

Portal tanısal

Çok fazlı BT çalışmasında intravenöz kontrast sonrası yumuşak doku bileşenlerinin boyanması ve kitlenin böbrek, kolon, psoas, aorta ile vena kava gibi komşu yapılara göre ilişkisi ve kitle etkisi değerlendirilir.

Önerilen pencereler: Batın (G 400 / M 50), Yumuşak doku (G 400 / M 40).

BT bulguları

• Makroskopik yağ içeren kitle — renal hilus çevresi veya pararenal alanda normal yağ dağılımını bozan, lobüle ve genişleyici retroperitoneal oluşum.
• Atipik septalar — yağlı doku içinde 3 mm'den kalın, düzensiz veya nodüler yumuşak doku bantları olup hafif veya değişken kontrastlanma gösterebilir.
• Yağ dışı nodül — tümörün yağ baskın bölümlerinden farklı atenüasyonda, kontrastlanan fokal yumuşak doku bileşeni; dediferansiye odak olasılığını artırır.
• Retroperitoneal köken işareti — böbrek ve kolonun tümörün dışına itilmesi, kitlenin bu organlardan kaynaklanmak yerine retroperitoneal kompartımandan büyüdüğünü destekler.
• Uzunlamasına yayılım — tümörün böbrek üstü seviyeden pelvise doğru retroperitoneal fasya planları boyunca devam etmesi görülebilir.
• Komşu yapı ilişkisi — kitlenin böbrek, üreter, psoas kasları ve büyük damarlara göre konumu, kitle etkisi, sarılması veya invazyon düzeyi tariflenir.
• Nekroz, hemoraji veya nadir kalsifikasyonlar yağlı kitle içinde görülebilir; ancak kesin alt tip tanısı için histopatoloji esastır.

Ölçütler ve sınıflamalar

WHO 5. baskı histolojik alt tipleri ve görüntüleme ilişkisi

WHO 2020 sınıflandırmasına göre liposarkomlar iyi diferansiye, dediferansiye, miksoid/round cell, pleomorfik ve undiferansiye pleomorfik alt tiplere ayrılır. Retroperitoneal lokasyonda iyi diferansiye ve dediferansiye tipler en sık görülenlerdir. İyi diferansiye tip genellikle makroskobik yağ ve kalın septalarla, dediferansiye tip ise buna ek belirgin yumuşak doku komponenti ile izlenir. Miksoid ve pleomorfik tiplerde makroskobik yağ daha az veya hiç görülmez. Görüntüleme alt tipleri düşündürebilir ancak kesin tanı için histopatoloji esastır. Retroperitoneal liposarkomlarda iyi diferansiye ve dediferansiye tipler özellikle önemlidir. İyi diferansiye tip yağ baskın, septalı kitle; dediferansiye tip ise buna eşlik eden belirgin yağ dışı solid bileşen gösterebilir. Miksoid ve pleomorfik tipler daha az tipik makroskopik yağ görünümü verebilir. BT bulguları alt tipi düşündürür, histopatolojinin yerini tutmaz.

Normalde

Perirenal boşluk önden Gerota, arkadan Zuckerkandl fasyası ile sınırlı olup böbrek, adrenal bez, üreter ve vasküler yapıları içerirken; posterior pararenal alan yalnızca yağdan oluşur. Normal retroperitoneal organların ve büyük damarların kitlenin önüne doğru displasması, lezyonun bu yapılardan değil primer olarak retroperitoneal boşuktan kaynaklandığını gösteren güçlü bir göstergedir.

Ayırıcı tanı

Renal anjiyomiyolipom

renal korteksten dışa doğru uzanan parankimal pençe veya renal hilusla devamlılık böbrek kökenini destekler.

Adrenal miyelolipom

yağ ve yumuşak doku bileşenleri adrenal bez merkezinde birleşir; bez kitlenin dışında itilmiş görünmez.

Retroperitoneal lipom

homojen yağ görünümü ve ince iç yapılar benign lipomu düşündürür; belirgin düzensiz septa veya kontrastlanan nodül şüpheyi artırır.

Retroperitoneal lipomatozis

yağ artışı tek bir kapsüllü veya lobüle kitle yerine yaygın ve organlar arası dağılır.

Ekstragonadal teratom

makroskopik yağla birlikte kistik içerik ve farklı olgun doku elemanlarına ait kalsifik yoğunluklar görülebilir.

Dediferansiye liposarkom

yağ içeren kitleye bitişik belirgin solid, kontrastlanan bileşen alt tip ayrımında öne çıkar.

Tuzaklar

• Normal retroperitoneal yağla benzer atenüasyondaki tümör kenarı tek aksiyel kesitte kaybolabilir; renal fasya ve komşu organların yer değiştirmesini kraniokaudal kesitlerde izleyerek uzanımı belirleyin.
• Böbrek yakınındaki yağlı kitleyi otomatik olarak liposarkom saymayın; parankimden çıkan pençe biçimi ve renal damarlarla ilişkisi anjiyomiyolipom lehine olabilir.
• Yağlı kitlenin yalnız baskın bölümünü örnekleyen bir görüntü, küçük solid bileşeni saklayabilir; tüm hacmi ve özellikle yumuşak doku nodüllerini gözden geçirin.
