Özet

Septik artrit, akut ağrı ve şişlikle başvuran hastada eklem boşluğunun enfeksiyonudur; hızla kıkırdak hasarına yol açabilir ve komşu osteomiyelitle ilişkili olabilir. BT erken enfeksiyon için özgül bir test değildir; kontrastlı inceleme eklem efüzyonunu, sinovyal kontrastlanmayı ve drenaj gerektirebilecek periartiküler koleksiyonu haritalamaya yardımcı olur. Sinovyal kalınlaşma ve efüzyon enfeksiyon dışı artritlerde de görüldüğünden BT tek başına tanı koydurmaz; tanısal temel eklem sıvısının değerlendirilmesidir. Erken kemik iliği enfeksiyonu BT'de görünmeyebileceği için negatif BT, güçlü klinik şüphesi olduğunda hastalığı dışlamaz.

Faz ve pencere

Venöz tanısal
Diz düzeyinde intravenöz kontrast sonrası venöz yumuşak doku değerlendirmesinde eklem efüzyonu, sinovyal kalınlaşma ve kontrastlanma, periartiküler yağ planlarında bulanıklık, flegmon veya çevresel kontrastlanan koleksiyon aranır. Sinovyal kontrastlanma özgül değildir; kontrastsız seride bu yumuşak doku yayılımını ve apse duvarını karakterize etmek yeterli olmaz.
Kemik algoritması tanısal
Kontrastlı serinin kemik rekonstrüksiyonlarında marjinal erozyon, subkondral kortikal düzensizlik, periost reaksiyonu ve geç dönemde kemik destrüksiyonu incelenir; erken kemik iliği osteomiyeliti BT ile dışlanamaz.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • Eklem efüzyonu — suprapatellar ve parapatellar ceplerde distansiyon; tek başına enfeksiyon kanıtı değildir.
  • Sinovyal kalınlaşma ve kontrastlanma — sinovyumun belirginleşmesi inflamatif süreçleri gösterir; inflamatif süreçler ile non-enflamatuar ödem ayırımında kontrastlı görüntülemede difüz kontrastlanma kullanılır.
  • Periartiküler yağ planı inflamasyonu — eklem çevresinde retikülasyon ve yumuşak doku şişliği enfeksiyonun çevreye yayılımını gösterebilir.
  • Periartiküler apse — çevresel kontrastlanan, merkezi düşük atenüasyonlu sınırlı koleksiyon; kas, bursa veya cilt altı kompartımanında yer alabilir.
  • Marjinal kemik erozyonu — femoral kondil veya tibial plato kenarında fokal kortikal kayıp; geç dönem eklem enfeksiyonu ya da eşlik eden osteomiyelitle ilişkili olabilir.
  • Periost reaksiyonu ve kemik destrüksiyonu — komşu osteomiyelit ilerlediğinde korteks ve periostta değişiklikler belirginleşebilir.
  • Eklem içi gaz — girişim, açık yaralanma veya gaz oluşturan enfeksiyon bağlamında değerlendirilir; tek başına patojeni belirlemez.

Normalde

Septik artritte BT'de eklem efüzyonu ve marjinal erozyon görülebilir. Kontrastlı BT'de sinovyal kalınlaşma ve efüzyon inflamatuvar süreci gösterir; ancak benign ödem, kristal artrit ve enfeksiyon arasında kesin ayrım yapamaz.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Kristal artritler de efüzyon ve sinovite neden olabilir; kesin ayırım için eklem sıvısı analizi ve özel görüntüleme teknikleri gereklidir.
Romatoid veya başka inflamatuvar sinovit
yaygın sinovyal proliferasyon ve efüzyon gösterebilir; erozyonlar dağılımı ve kronik klinik bağlamıyla yorumlanır.
Travma sonrası ortaya çıkan eklem içi veya çevresindeki yağ-kan seviyesi, kırık şüphesini artıran önemli bir bulgudur.
Selülit veya periartiküler apse
enfeksiyon eklem dışı yumuşak dokuda merkezlenir; kapsül ve eklem aralığına uzanım ayrıca aranır.
Osteomiyelit
kemik merkezli destrüksiyon veya periost değişikliği öne çıkabilir; erken medüller yayılım MR'da daha görünür.

Tuzaklar

  • Efüzyon ile sinovyal kontrastlanmayı septik artrit için özgül saymayın; inflamatuvar ve kristal artritler benzer BT görünümü oluşturabilir.
  • Normal veya silik kemik BT'si erken osteomiyeliti dışlamaz; kemik iliği tutulumu ve kıkırdak hasarı için MR daha duyarlıdır.
  • Eklem içi gaz, pirojenik enfeksiyon veya travmatik artotomi bulgusu olabilir; klinik bağlam (travma, girişim) ile ilişkilendirilmelidir.
  • BT eklem sıvısının enfekte olup olmadığını kesinleştirmez; yalnızca enfeksiyonun yayılımını gösterir. Mikrobiyolojik tanı eklem sıvısının mikrobiyolojik incelenmesiyle konur.

Kesinleştirme eklem sıvısının incelenmesini gerektirir. Osteomiyelit için kemik bulgusu yoktur; kırık da tarif edilmemiştir.

    Periartiküler apse. Çevresel kontrastlanma gösteren sınırlı düşük atenüasyonlu koleksiyon apse morfolojisidir. Baker kisti tipik popliteal bursa konumunda düzgün sıvı kesesidir; menisküs kalsifikasyonu küçük yoğun odaktır; OCD subkondral kemik lezyonudur.
