Fifth metacarpal neck fracture

Eğitim amaçlıdır; hasta başında tanı ve tedavi kararının yerine geçmez.

Özet

Sık görülen bir metakarp kırığı olan 5. metakarp boyun kırığı, yumrukla sıkılmış ele doğrudan travma sonrası gelişebilir ve genellikle apeks dorsal açılanma gösterir. Tanıda ilk basamak grafidir; gizli metakarp kırıklarında erken BT tanıyı doğrulamaya yardımcı olabilir. Kırık hattının boyunla sınırlı olup olmadığı ve eklem içi uzanımı, yaralanmanın ekstraartiküler mi intraartiküler mi olduğunu tanımlamaya yardımcı olur. Metakarpal kırık değerlendirilirken radyolojik bulguların yanı sıra klinik muayenede parmakların rotasyonel hizası ve karşı el ile kıyaslama ayrıntılı olarak yapılmalıdır.

Okuma sırası

  1. 5. metakarp boynu. Sagittal oblik kemik kesitte 5. metakarp baş-boyun geçişini bulun ve kortikal halkanın nerede kesildiğini izleyin.
  2. Distal fragmanın yönü. Sagittal BT kesitlerinde 5. metakarp boyun kırığındaki palmar angülasyonu değerlendirin.
  3. MCP eklem yüzü. Kırık çizgisini distal uca kadar takip ederek metakarp başının eklem yüzüne ulaşıp ulaşmadığını kontrol edin.
  4. 5. CMC ve hamat. İnceleme alanında 5. metakarp tabanını, CMC eklemi ve komşu hamatı tarayarak eşlik eden kırıklı çıkığı dışlayın.

Kontrastsız tanısal
İlk inceleme el grafileridir. Grafilerde gizli kalan metakarp kırıklarında erken BT tanıya yardımcı olabilir; el ve el bileği MRI incelemesi de sıklıkla ilk grafilerden sonra yapılır.
Kemik algoritması tanısal
Sagittal/koronal oblik rekonstrüksiyonlar distal fragmanın palmar angülasyonunu ve MCP eklem subkondral yüzüyle ilişkisini; aksiyel kesitler ise kortikal parçalanmayı gösterir.

BT bulguları

  • Boyun korteksinde kesinti — metakarp başının hemen proksimalinde kırık hattı ve trabeküler uzanım.
  • Palmar angülasyon — 5. metakarp boyun kırığında palmar angülasyon sagittal BT kesitlerinde ölçülebilir.
  • Dorsal apeks açılanması — yumruk travmasına bağlı 5. metakarp boyun kırığında interosseöz kasların çekimiyle görülebilen açılanma.
  • 5. metakarp boyun kırığında distal fragmanın volar angülasyonu dorsal apeks açılanmasına karşılık gelir.
  • Kısalma ve translasyon — 5. metakarp başının komşu metakarp başlarına göre proksimal konumu ya da eksenden kayması.
  • MCP eklem uzanımı — kırık çizgisi başın subkondral yüzüne ulaşırsa eklem içi bileşen mevcuttur.
  • Komşu yaralanma olasılığı — Yumruk travmalarında 4. metakarp ve 5. metakarp tabanı-CMC eklemi yaralanmaları da görülebilir.

Normalde

Beşinci metakarp boynunun normal açılanması yaklaşık 15° olarak bildirilmiştir; ölçülen açı kullanılan yönteme göre değişebilir. Metakarpal kırık değerlendirilirken komşu sağlam metakarpallar ve karşı el ile boyut, eksendeşlik ve kısalma açısından kıyaslama yapılmalı, rotasyonel değerlendirmede ise klinik muayene esas alınmalıdır.

5. metakarp başı osteokondral kırığı
ayrışmış fragman subkondral eklem yüzündedir; yalnız boyun metafizinde kalmaz.
5. metakarp cisim kırığı
kırık çizgisi uzun diyafiz boyunca ilerler ve distal boyun konturu sağlam kalabilir.
Ters Bennett kırığı
lezyon boyunda değil 5. metakarp tabanı-CMC eklemindedir ve çoğunlukla eklem uyumsuzluğu eşlik eder.
Metakarpofalangeal eklem çıkıklarında rotasyonel hizasızlık ve eklem uyumsuzluğu gözlenir; klinik muayenede karşı el ile kıyaslama zorunludur.

  • Palmar angülasyonu aksiyel kesitte tek başına sınıflamayın; başın yönünü sagittal oblik rekonstrüksiyonda metakarp uzun eksenine göre izleyin.
  • Eklem içi bileşen şüphesinde — Articular yüzeydeki depresyon veya fragmanları ayırt etmek için ince kesitli çoklu planar yeniden yapılandırmalar gereklidir.
