Özet

Önceden skafoid kırığı geçirmiş ve bilek ağrısı süren hastada psödartroz, kemik deformitesi ve proksimal fragmanın olası osteonekrozu ayrı bileşenler olarak incelenir. Kontrastsız ince kesit BT, devam eden kırık hattını, sklerotik uçları, kistik değişikliği, çökme ve skafoid kaynaklı karpal artrozun yerleşimini gösterir. BT'de proksimal kutup sklerozu canlılık kaybını kanıtlamaz; kemik iliği ve perfüzyonla ilişkili değerlendirme için MRI daha uygundur. Tedavi edilmeyen skafoid kaynamaları, ileri evrede skafoid-nonunion-ilermiş-kollaps (SNAC) adı verilen öngörülebilir bir karpal çökme ve osteoartrit örüntüsüyle seyretmektedir.

Faz ve pencere

Kontrastsız tanısal
Skafoid boyunca devam eden kırık hattı, karşılıklı sklerotik psödartroz uçları, yuvarlak kistik boşluklar, volar kortikal kısalma ve proksimal kutupta skleroz ya da çökme aranır. İyotlu kontrast gerekmez; yoğunluk artışı BT'de tek başına osteonekroz tanısı koydurmaz.
Kemik algoritması tanısal
Skafoid uzun eksenine göre eğik koronal ve sagittal görüntülerde kemik köprüsü yokluğu, humpback biçimi, proksimal fragman konturu ve skafoid-lunatum hizası değerlendirilir; radyoskafoid ve kapitolunat eklem yüzlerindeki dejenerasyonun dağılımı izlenir.

Önerilen pencereler: Kemik (G 1800 / M 400).

BT bulguları

  • Psödartroz hattı — eski kırık seviyesinde iki fragman arasında devam eden yarık ve trabeküler köprü yokluğu; karşılıklı kenarlarda yoğun skleroz eşlik edebilir.
  • Sklerotik ve rezorptif değişiklikler — psödartroz kenarlarında skleroz ve proksimal kutupta rezorpsiyon gibi metabolik/klinik değişiklikler gözlenebilir.
  • Humpback konfigürasyonu — skafoid belinde volar korteks kısalır, distal fragman fleksiyona döner ve skafoid uzun eksen konturu açılanır.
  • Skafoid-lunatum malalignment — skafoid deformitesine lunatumun dorsal rotasyonu eşlik edebilir; lunokapitat eklem ekseni de kontrol edilir.
  • Proksimal kutup sklerozu — distal fragmana kıyasla yoğun görünüm osteonekroz şüphesini artırır, ancak BT'nin kemik yoğunluğu canlılık ölçümü değildir.
  • Proksimal fragman çökmesi — eklem yüzünde düzleşme, basamaklanma veya hacim kaybı gelişmiş yapısal bozulmayı gösterir.

Ölçütler ve sınıflamalar

Vender SNAC evrelemesi
Evre I: distal skafoid fragmanı ile radial stiloid arasındaki eklemde dejenerasyon. Evre II: Evre I’e ek olarak proksimal skafoid fragmanı ile kapitat arasındaki skafokapitat eklemde dejenerasyon. Evre III: radioskafoid, skafokapitat ve kapitolunat eklemler birlikte etkilenir. Radyolunate eklemi çoğunlukla korunur — hastalık ilerlediğinde bile radyolunate eklem nadiren tutulur. Bu evreleme skafoid nonunionuna bağlı SNAC içindir; osteonekroz tek başına SNAC evresi değildir.

Normalde

Normal karpal hizalama — lateral görünümde kapitat, radius ve pisiform arasındaki eksenel uyumluluk korunur. Normal lunatum kapitat başıyla uyumludur; radyoskafoid ve kapitolunat eklem aralıkları korunur. Karşı tarafla skafoid kutuplarının yoğunluğunu, bel uzunluğunu ve lunatum-kapitat eksenini kıyasla.

Ayırıcı tanı

Skafoid malunionu
kırık hattı iyileşmiş durumdadır; fibroz veya sklerotik psödartroz yarığı yerine kemik bütünlüğü izlenir.
Akut skafoid fraktürü
travma sonrası skafoidde belirgin kırık hattı ve çevresinde ödema/hematoma ile lokal ağrı ve tenderness eşlik eder.
Kienböck hastalığı
skleroz ve çökme lunatumda merkezlenir; skafoidde kırık aralığı beklenmez.
Skafoid osteonekrozu
proksimal kutup yoğunlaşması ve olası çökme görülebilir; BT kemik morfolojisini gösterirken canlılığın değerlendirilmesinde MRI kullanılır.
Reumatoid artrit
inflamatuar süreçlerde eklem yüzlerinde yaygın erozyon ve sinovyal hipertrofi izlenir; skafoidde travmaya bağlı psödartroz yarığı bulunmaz.

Tuzaklar

  • Proksimal fragmanın sklerozunu kesin osteonekroz diye raporlama; BT yoğunluğu dolaşımı göstermez ve MRI bulguları ile birlikte yorumlanmalıdır.
  • Humpback görünümünü standart sagittal kesitte skafoidin oblik seyri abartabilir; deformiteyi skafoid uzun eksenine paralel koronal ve sagittal kesitlerde doğrula.
  • SNAC artrozunu tek bir “karpal artroz” ifadesine indirgeme; distal fragman-radial stiloid, skafokapitat ve kapitolunat eklemleri, ileri evrede de radioskafoid eklem tutulumunu ayrı ayrı incele.
