Özet

Femur başı osteonekrozu genç ve orta yaşlı erişkinlerde steroid maruziyeti, alkol kullanımı, hemoglobinopati veya travma bağlamında görülebilir; erken evre hastalık asemptomatik de olabilir. BT, erken kemik iliği hastalığını dışlamak için uygun değildir; MR erken tanıda öne çıkar, ince kesit BT ise subkondral kırık ve baş çökmesini göstermede özellikle yararlıdır. Kırık hattı ya da kontur kaybının fark edilmemesi evreyi olduğundan düşük göstermeye ve eklem yüzü hasarının gözden kaçmasına yol açabilir. Subkondral kırığı saptamak ve lezyon yayılımını belirlemek amacıyla BT istenebilir; ancak tanı ve evrelemede MR daha sık kullanılır.

Okuma sırası

  1. Femur başları. Koronal kemik penceresinde iki femur başını aynı anda bul; süperior yük taşıyan yüzeylerin küreselliğini ve sağ-sol yoğunluk farkını gözle.
  2. Femur başı subkondral segmenti. Şüpheli başın içindeki kıvrımlı sklerotik sınırı kesitler boyunca izle; sınırın bir segmenti çevreleyip çevrelemediğini göster.
  3. Süperior subkondral kemik. Subkondral kortekste eklem yüzüne paralel ayrışma veya kresent çizgisi arayarak nekrozun kırık evresine geçip geçmediğini belirle.
  4. Femur başı konturu. Femur başı üst kontüründeki çökme ölçülür; ARCO evrelemesi için çökme derinliği ve yayılımı belirlenir.
  5. Kalça eklem aralığı ve karşı kalça. Son olarak eklem aralığı, asetabular çatı ve karşı kalçada ikincil kıkırdak-kemik hasarı ya da eş odak ara.

Faz ve pencere

Kontrastsız tanısal
Femur başında coğrafi sınırlı skleroz, serpijinöz subkondral kırık hattı (kresent) ve eklem yüzü çökmesi aranır.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Coğrafi serpijinöz skleroz — femur başının ağırlık taşıyan bölümünde nekrotik segmenti çevreleyen kıvrımlı yoğun kenar.
  • Subkondral kresent — eklem yüzüne paralel ince yarık veya kortikal kesinti; BT subkondral kırığı göstermede duyarlıdır.
  • Eklem yüzü çökmesi — femur başı süperior kontürünün düzleşmesi veya basamaklanması ile karakterizedir.
  • Segmental femur başı deformasyonu — baş-boyun bileşkesine doğru uzanabilen fokal kontur kaybı.
  • Nekrotik alan çevresinde sklerotik sınır — canlı ve etkilenmiş kemik arasındaki coğrafi geçişi belirginleştirebilir.
  • Sekonder osteoartrit — ilerlemiş olguda eklem aralığı daralması, asetabular subkondral skleroz/kistler ve marjinal osteofitler.
  • İki taraflı veya çok odaklı kemik tutulumu — karşı kalça ve görüntülenen diğer kemiklerde benzer odaklar sistemik risk bağlamını destekleyebilir.

Ölçütler ve sınıflamalar

ARCO 2019
Evre I: radyografi ve BT normal, MR veya sintigrafi anormal. Evre II: radyografide skleroz/osteoliz/kistler, subkondral kırık veya düzleşme yok. Evre III, radyografide kresent (subkondral kırık) bulgusuyla tanımlanır. Evre IV: eklem aralığı daralması, asetabular değişiklik veya eklem yıkımıyla osteoartrit.

Normalde

Normal femur başı kontürü küreseldir; subkondral kemikte serpiginöz sklerotik sınır veya kırık hattı yokluğu normaldir. Femur başı süperior eklem yüzü kontürü, subkondral çizgi devamlılığı ve serpijinöz sklerotik sınırın varlığı değerlendirilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

İleri kalça osteoartriti
daralma ve osteofitler eklem genelinde dağılım gösterir; fokal femur başı nekroz sınırı beklenmez.
Femur başı travmatik osteokondral kırığı
travma öyküsüyle uyumlu keskin parça/eklem yüzü basamağı ve eşlik eden asetabular ya da femur boynu kırığı araştırılır.

Tuzaklar

  • Normal veya belirsiz BT osteonekrozu dışlamaz; kemik iliği sinyal değişikliği ve erken nekrotik sınır için MR daha duyarlıdır.
  • Kresenti otomatik olarak osteonekroz kabul etme: subkondral yetersizlik kırığı da çökme yapabilir; klinik bağlam ve MR bant morfolojisiyle ayır.
  • Tek bir oblik aksiyel kesitte görünen kontur basamağını çökme sayma; koronal ve sagittal yeniden biçimlendirmelerde devamlılığını doğrula.
  • Bilateral hastalıkta belirgin taraftaki çökme, karşı taraftaki daha erken lezyonu görünmez kılabilir; iki femur başını ayrı ayrı tara.
