Özet

Malignite öyküsü bulunan hastada ekstremitedeki özgül olmayan ağrı ek tanısal inceleme gerektirir; tek ve sınıflandırılamayan kemik lezyonunda biyopsi gündeme gelebilir. BT kemik matriksini ve kortikal kesintileri değerlendirmeye katkı sağlar; kırık riskiyle ilişkili aksiyel kortikal tutulum BT görüntülerinde ölçülebilir. BT kemik matriksini değerlendirmede tercih edilir; MR lezyonun medüller kanal ve komşu yumuşak dokulara uzanımını gösterir. Femoral kemik metastazı patolojik kırığa yol açabilir; aksiyel kortikal tutulumu 30 mm veya daha fazla olan lezyonlarda kırık riski daha yüksektir.

BT bulguları

  • Litik medüller odak — trabeküler yapı kaybolur, medulla düşük dansiteli görünür ve endosteal sınır oyulabilir; özellikle kortekse yaklaşan yayılımı not et.
  • Sklerotik odak — femoral kemik metastazında artmış kemik dansitesi ve sklerotik alanlar görülebilir.
  • Mikst matriks — aynı lezyonda litik ve osteoblastik kemik değişiklikleri birlikte görülebilir.
  • Lezyonun iç kortekse temasıyla oluşan kortikal incelme veya kayıp, yapının zayıflamasını ve kırık riskini belirleyen önemli bir bulgudur; çok düzlemli görüntüleme ile değerlendirilmelidir.
  • Kortikal destrüksiyon — medüller lezyondan dış kontura uzanan düzensiz kortikal kesinti, yapısal zayıflık ve kırık riskini artıran lokal yayılımı gösterir.
  • Patolojik kırık — tümör odağı içinden geçen kortikal/medüller kırık hattı ve yer değiştirme ayrıca tanımlanır.
  • Ekstraosseöz yumuşak doku uzanımı — MR, lezyonun komşu yumuşak dokulara uzanımını değerlendirmeye yardımcı olur.
  • Çok odaklı kemik hastalığı — femoral metastaz, çoklu kemik metastazlarının bulunduğu olgularda daha sık görülmüştür.

Normal BT ile kıyasla

Tuzaklar

  • Tek bir litik odağı otomatik olarak metastaz kabul etme; bilinen primer tümör, önceki incelemeler, diğer kemik odakları ve agresif primer kemik tümörü olasılığını birlikte değerlendir.
  • Yeni sklerotik lezyonlar tedaviden kaynaklanan yalancı ilerleme (pseudoprogression) olabilir; ekstraosseöz progresyon veya kemikte ilerleyici litiz yoksa, bu durum progresyon olarak kabul edilmemelidir.
  • Mirels veya kortikal tutulum eşiğini kırık tanısı gibi sunma; bunlar metastatik lezyonda kırık riskini sınıflamak içindir ve klinik/ortopedik bağlam gerektirir.

İyi huylu kıkırdak lezyonları ile düşük dereceli kondrosarkom arasında klinik ve görüntüleme özellikleri büyük örtüşme gösterebilir; diferansiyel tanı için histopatolojik doğrulama gerekebilir.
