Özet

Akut osteomiyelit, hematojen yayılım, doğrudan inokülasyon veya komşu yumuşak doku enfeksiyonunun kemiğe ilerlemesiyle gelişir; çocuklarda en sık uzun kemik metafizi tutulurken, erişkinlerde enfeksiyonlar daha çok komşu yumuşak doku kaynaklıdır. Erişkin uzun kemik enfeksiyonlarında komşu yumuşak dokudan yayılım veya doğrudan inokülasyon önemlidir. BT kortikal yıkımı, periostal kemik oluşumunu, gazı ve kemik anatomisini gösterir; erken kemik iliği enfeksiyonunu saptamada MR daha duyarlıdır. Kontrastlı BT, seçilmiş olgularda kemik çevresi yumuşak doku uzanımı ve koleksiyon sınırlarını gösterebilir. Normal erken BT enfeksiyonu dışlamaz; subperiostal veya yumuşak doku apsesi ile ilerleyici kemik hasarı gözden kaçabilir.

Faz ve pencere

Kontrastsız tanısal
Kemik algoritmasında medüller boşlukta silik heterojenlik, kortikal düzensizlik veya erozyon, periostal yeni kemik ve kemik/yumuşak doku içinde gaz aranır; erken olguda korteks ve trabeküller normal kalabilir.
Portal tanısal
İV kontrast sonrası yumuşak doku penceresinde kemik çevresi ödemli kalınlaşma, subperiostal ya da intramüsküler koleksiyon ve çevresel kontrastlanan apse duvarı değerlendirilir; kemik iliği enfeksiyonunu dışlamak için tek başına yeterli değildir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Metafizde kemik iliği heterojenliği — erken dönemde belirgin olmayabilir ve tek başına osteomiyelit tanısını doğrulamaz.
  • Kortikal erozyon — medüller odaktan dış kortekse uzanan düzensiz kesinti enfeksiyon lehine yapısal hasarı gösterir.
  • Periostal yeni kemik — korteks boyunca solid ya da lameller kemik oluşumu komşu inflamasyona eşlik edebilir.
  • Subperiostal koleksiyon — korteksi periosttan ayıran sıvı dansiteli alan kontrastlı seride çevresel boyanabilir.
  • Yumuşak doku şişliği — kemik çevresi yağ planlarının silinmesi ve kas/fasya boyunca asimetrik kalınlaşma.
  • İntraosseöz veya yumuşak doku gazı — açık yara, cerrahi ya da gaz oluşturan enfeksiyon bağlamında anlam kazanır.
  • Kortikal destrüksiyon boyunca komşu apse — enfeksiyon odağı ile ekstraskeletal koleksiyon arasında anatomik devamlılık bulunabilir.

Normalde

Kemik karşılaştırmasında BT, kemik yapısındaki ince değişiklikleri, periostal reaksiyonu ve kortikal yıkımı gösterebilir. Karşı tarafla karşılaştırmalı radyografiler negatif olsa bile klinik kuşku sürüyorsa ileri değerlendirme için BT veya MR kullanılabilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Travma öyküsü, akut osteomiyelitin ayırıcı tanısında önemlidir; osteomiyelitli hastada akut yaralanma öyküsü bulunmayabilir.
Septik artrit
eklem efüzyonu ve eklem yüzlerinde değişiklik ön plandadır; kemik içindeki metafizer odak eşlik etmeyebilir.

Tuzaklar

  • Erken akut enfeksiyon BT'de normal görünebilir; klinik kuşku sürüyorsa normal korteks ve trabekülleri hastalığı dışlayan bulgu gibi sunma, MR'ın kemik iliği duyarlılığını belirt.
  • Çocuk metafizinde fizis komşuluğundaki normal damar kanalları kortikal açıklık izlenimi verebilir; devam eden kesitlerde ve karşı tarafla karşılaştır.
  • Periostal reaksiyon yalnızca enfeksiyonla ilişkili değildir; travma, kemik tümörleri ve diğer inflamatuar süreçlerde de görülebilir, bu nedenle klinik bağlam ve eşlik eden kortikal değişikliklerle birlikte değerlendirilmelidir.
