Özet

Sakral kırıklar yüksek enerjili pelvis travmasında posterior halka hasarının önemli bir bölümünü oluşturur; yaşlı veya osteoporotik hastada düşük enerjili yetmezlik kırığı biçiminde de ortaya çıkabilir. İnce kesit BT kırık yönünü, deplasmanı, foraminal/kanal uzanımını ve SI eklem ile anterior halka bağlantısını gösterir. Kırık hattının anatomik seviyesi nörolojik etkilenme ve spinopelvik bütünlük açısından anlam taşır; iki taraflı vertikal çizgiler arasında gizli transvers bileşen aranmalıdır. Foraminal bası, deplasman veya U/H biçimli ayrışma gözden kaçarsa sinir kökü yaralanması ve instabilitenin görüntüleme tanımı eksik kalır.

Faz ve pencere

Kontrastsız tanısal
Aksiyel kemik penceresinde sakral korpus, ala, foraminalar ve kanal boyunca kortikal kırık çizgisi, impaksiyon, fragman ve kanal/foramen daralması aranır; yumuşak doku penceresinde presakral hematom da görünür. Kırık morfolojisi için kontrast fazı gerekmez.
Kemik algoritması tanısal
Koronal rekonstrüksiyon vertikal alar/transforaminal çizgileri ve hemipelvisler arası seviye farkını, sagittal rekonstrüksiyon transvers bileşen ile kifoz veya öne-arkaya translasyonu gösterir; aksiyel kesitlerde foraminal ve kanal uzanımı tamamlanır.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Kırık hattı yönelimi — vertikal ala, transforaminal, santral veya transvers uzanımı anatomik yapı adlarıyla tarif edin.
  • Foramen ilişkisi — çizginin sakral foramenden geçip geçmediğini, foramen çapında daralma ve fragman basısını belirleyin.
  • Santral kanal tutulumu — medial sakral korpus veya posterior duvar fragmanının kanala uzanımını izleyin.
  • Deplasman ve impaksiyon — koronal dikey basamak, aksiyel ön-arka ötelenme, açılanma ve ezilme morfolojisini tarif edin.
  • Transvers bileşen — iki taraflı vertikal kırık arasında sakral korpusu kesen yatay çizgiyi sagittal ve aksiyel düzlemde arayın.
  • Spinopelvik ayrışma — U veya H biçimli birleşik kırıkta pelvis ve alt sakrumun eksial iskeletten kopukluğunu gösteren örüntüdür.
  • Halka ve yumuşak doku eşliği — SI eklem, iliak kemik, pubik ramuslar, presakral hematom ve pelvis içi yaralanmayı birlikte gözden geçirin.

Ölçütler ve sınıflamalar

Denis anatomik zonları
Denis sınıflaması, sakral kırık hattının sakral foraменlere göre konumuna dayanarak nörolojik hasar riskini tahmin etmeyi amaçlar. Denis sınıflaması mekanik stabiliteye dayanmaz; yalnızca nörolojik risk tahmini için sakral foraменlere göre kırık konumunu kullanır.
AOSpine sakral yaralanma sınıflaması
AOSpine sakral yaralanma sınıflaması, morfoloji, nörolojik durum ve hasta/yaralanma değiştiricilerini birlikte kodlar; BT nörolojik muayenenin yerini tutmaz ancak hasta muayene edilemediğinde dolaylı bir bulgu sağlar.

Normalde

Normal sakrumda korpus ve ala korteksleri kesintisizdir; bilateral sakral foraminalar düzenli aralıklarla sıralanır ve kanal konturu korunur. Aynı seviyede karşı ala, foraminal çap, SI eklem yüzü ve sakral korpus hizasını karşılaştırın; asimetrik skleroz veya anatomik varyantı gerçek kortikal kırık çizgisinden ayırın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

alada vertikal ince çizgi ve skleroz eşlik edebilir; akut travma bağlamı veya belirgin deplasman bulunmayabilir.
Sakroiliit
SI eklem yüzlerinde erozyon, skleroz ya da eklem aralığı değişikliği vardır; sakral kortekste travmatik kesinti beklenmez.
Metastaz veya primer kemik lezyonları
sakrumda yaygın patolojilerdir ve ayırıcı tanıda radyolojik özelliklerin klinik bağlamla değerlendirilmesi gerekir.
Osteomiyelit
sakral enfeksiyonlarda perivertebral koleksiyon ve kas infiltrasyonu görülebilir; travmatik olmayan enfeksiyon bulguları kırık çizgisinden farklılaşmalıdır.
Sakral foraminal varyant veya damar kanalı
düzgün kortike kenarlı, tipik yerleşimli kanal komşu kesitlerde devam eder.

Tuzaklar

  • İki taraflı vertikal alar çizgileri gördüğünüzde sagittal rekonstrüksiyonları atlamayın; aradaki transvers hat spinopelvik ayrışma örüntüsünü değiştirebilir.
  • Osteoporotik hastada ince, deplase olmayan çizgi BT'de silik olabilir; klinik kuşku sürerken BT negatifliğini kesin dışlama gibi sunmayın.
  • Foraminalara komşu kırık çizgisini foramen içine uzanım olarak otomatik sınıflamayın; ardışık aksiyel kesitlerde kortikal hattın kök kanalını kesip kesmediğini doğrulayın.
  • Pelvik rotasyon sakrumda yalancı asimetri oluşturabilir; koronal-sagittal düzlemleri ve karşı tarafı kullanarak gerçek basamağı ayırt edin.
