Özet
Ateş ve tek taraflı kalça ya da pelvik ağrısı olan hastada septik sakroiliit, eklem dışındaki komşu yumuşak dokulara ve kemiğe uzanabilen bir enfeksiyondur. BT çoğu kez acilde karın-pelvis ağrısının başka nedenini araştırırken çekilir; kontrastlı görüntülerde eklem önündeki yağ planı, kapsül ve komşu kaslar dikkatle taranmalıdır. BT'de septik sakroiliit bulguları görülmeyebilir; enfeksiyon şüphesinde tercih edilen görüntüleme yöntemleri arasında intravenöz gadolinyum kontrastlı MR bulunur. Tanının atlanması iliak veya piriformis kasına uzanan apseyi ve kemik destrüksiyonunu gözden kaçırabilir.

BT bulguları
- Perilesyonel steatit ve yumuşak doku ödemi — enfekte eklem çevresinde yağ planında infiltrasyon ve komşu kaslarda şişlik izlenir.
- Eklem effüzyonu ve kapsül tutulumu — enfekte eklemlerde sinovyal sıvı birikimi ve kapsülün inflamatuar reaksiyonu izlenebilir.
- İliakus kasında şişme — etkilenen tarafta kas hacmi artar ve komşu yağ planı silinir.

Normalde
Normalde iki sakroiliak eklemin önündeki yağ planları temiz, iliakus ve piriformis kaslarının hacim ve dansitesi simetriktir; eklem aralıkları düzenli ve korteksler kesintisiz görünür. Aynı aksiyel seviyede karşı tarafı referans alarak yağ bulanıklığını, kapsül konturunu ve kas asimetrisini kıyaslayın.
Normal BT ile kıyasla
Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı
- Spondiloartritte sakroiliak tutulum tek ya da iki taraflı olabilir; ankilozan spondilitte genellikle iki taraflı ve simetrikken diğer alt gruplarda bilateral tutulum daha seyrek, tek taraflı veya asimetrik tutulum daha sık görülür. Erozyon, subkondral skleroz ve ankiloz gibi yapısal değişiklikler klinik bilgilerle birlikte değerlendirilir.
- Sakroiliak osteoartrit ve dejeneratif süreçlerde eklem yüzlerinde osteofit ve vakum fenomeni öne çıkar; enflamatuar yağ infiltrasyonu bu tabloya ait değildir.
- İliak veya sakral stres kırığı
- eklem yüzünü izlemeyen ince kırık hattı ve kemik içinde sklerotik reaksiyon enfeksiyondan ayrılır.
- Osteitis condensans ilii
- iliak tarafta iyi sınırlı üçgensi skleroz vardır; kapsül bombeleşmesi veya kas apsesi oluşturmaz.
- Komşu pelvik apse değerlendirilirken enfeksiyon kaynağının sakroiliak eklem içi mi yoksa eklem dışı sistemik/barsaksal mı olduğu ayrımına dikkat edilir.
Tuzaklar
- Pelvis BT'si yalnız barsak ve jinekolojik organlara odaklanırsa sakroiliak eklem önündeki küçük yağ infiltrasyonu fark edilmeyebilir; iki eklemin çevresini ayrıca gözden geçirin.
- Erken olguda kortikal erozyon bulunmaması enfeksiyonu dışlamaz; kas şişliği ve kapsül bombeleşmesi kemik değişikliğinden önce ortaya çıkabilir.
- Kas asimetrisini tek başına apse diye adlandırmayın; koleksiyon için sıvı merkezi ve kontrastlanan sınır arayın, şüphe sürüyorsa MR ile tamamlayın.
- BT’de periartiküler erozyonlar görülebilir; ancak BT’nin normal olması septik sakroiliiti dışlamaz. Klinik şüphe sürüyorsa MR ile değerlendirme yapılır.

Kontrastlı BT'de sakroiliak eklem komşuluğundaki iliakus içinde çevresel kontrastlanan sıvı odağı görülüyor. Bu bulgu neyi düşündürür?

İliakus apsesi. Sıvı yoğunluklu merkez ve çevresel kontrastlanma kas apsesini destekler. Subkondral kist kemik yüzüne bitişiktir; osteofit kemik çıkıntısı, stres kırığı ise çizgisel kortikal/medüller bulgudur.
