Özet

Yüksek enerjili servikal travmada korpusun posterior duvarı veya posterior elemanlardan kopan kemik parçası kanala ilerleyebilir. İnce kesit kontrastsız BT, fragmanın kaynağını, yönünü ve kanal konturuna etkisini hızlı biçimde ortaya koyar. BT kemik kanal daralmasını ve travmanın kemik morfolojisini gösterir; spinal kord yaralanmalarının değerlendirilmesinde MR ek bilgi sağlar. Bası ve eşlik eden kırık örüntüsü atlanırsa instabilite ile nörolojik yaralanmanın anatomik zemini eksik raporlanabilir.

Kontrastsız tanısal
Posterior korpus duvarından kanala doğru yer değiştiren yüksek dansiteli kemik fragmanı, lamina ya da faset parçası ve kanalın kemik sınırındaki daralma izlenir; intravenöz kontrast gerekmez.
Kemik algoritması tanısal
Aksiyel kesitler fragmanın kanal içindeki izdüşümünü; sagittal ve koronal rekonstrüksiyonlar kırık hattının vertebra ve posterior elemanlar boyunca devamını gösterir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Retropulsiyon — vertebra korpusunun kırılmış posterior duvarından kemik parça spinal kanala doğru yer değiştirir.
  • Kanal konturunda fokal daralma — fragmanın olduğu seviyede kemik kanalın ön-arka çapı veya kesitsel alanı komşu seviyeye göre azalır.
  • Posterior eleman fragmanı — lamina, faset ya da spinöz çıkıntı parçası kanalın arka veya yan sınırına uzanabilir.
  • Kırık hattı devamlılığı — sagital rekonstrüksiyonda korpus kırığı endplate ve posterior kortekse kadar izlenebilir.
  • Burst kırığında kemik fragmanları spinal kanala uzanabilir ve ciddi nöral hasarla ilişkili olabilir.
  • Eşlik eden hizalanma kaybı — translasyon, segmental kifoz veya faset kilitlenmesi kemik basısına eşlik edebilir.
  • Kord dokusu sınırı — BT kemik fragmanı ve kanal geometrisini gösterir, kord ödemi ya da kontüzyonunu güvenle dışlamaz.

Normalde

Normal servikal kanalda korpusların posterior korteksi düzgün bir ön sınır, laminalar ve ligamentöz ark ise arka sınırı oluşturur; kanal boyunca kemik konturunda ani çıkıntı bulunmaz. Yaralı seviyeyi üst ve alt komşu seviyelerle, mümkünse karşı taraf faset ve lamina konturlarıyla kıyaslayıp daralmanın fragmanla aynı noktada olup olmadığını izleyin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

  • Kanal daralmasını yalnız tek aksiyel görüntüde ölçüp kırık hattını kaçırma; fragmanın korpustan mı fasetten mi geldiğini sagittal ve koronal planda izle.
  • Kanal içindeki kemik parçanın varlığını kord yaralanmasının derecesi gibi sunma; BT yumuşak kord dokusunu yeterince karakterize etmez.
  • Küçük retropulse fragmanı osteofit sanma; akut kırık kenarı keskin ve düzensiz olabilir, komşu kortekste de kesinti aranır.
