Özet

Ankilozan spondilit veya DISH nedeniyle rijitleşmiş omurgada düşük enerjili travma bile uzun kaldıraç kolu boyunca kırığa yol açabilir. İnce kesit kontrastsız BT, transdiskal ya da transvertebral hattı, posterior eleman uzanımını ve deplasmanı gösterir; inceleme tüm omurgayı kapsamalıdır çünkü ek düzey yaralanmalar olabilir. Kırık küçük deplasmanlı görünse bile mekanik olarak ciddi olabilir ve ikincil yer değiştirme nörolojik hasara neden olabilir. Yalnız ağrılı seviyeye odaklanmak veya kırığı gözden kaçırmak ek yaralanmayı ve spinal kanal riskini atlatabilir.

Faz ve pencere

Kontrastsız tanısal
Tüm omurgada aksiyel kesitler ve sagittal/koronal kemik rekonstrüksiyonları; köprüleşmiş segmentte diskten veya vertebra korpusundan geçen yatay-oblik kırık hattı, posterior arkus uzanımı, faset ayrışması ve hizalanma bozukluğunu gösterir. Kırık haritalaması için BT’de yaralanma düzeyinin en az üç vertebra üstünü ve altını kapsayan iki düzlemli rekonstrüksiyon veya 3B görüntüleme önerilir.
Kemik algoritması tanısal
İnce kemik rekonstrüksiyonları köprüleşen osteofit veya sindesmofit içinden geçen çatlağı, küçük kortikal ayrışmayı ve kırığın disk-korpus-kanal boyunca devamını görünür kılar; tüm omurga kapsamı ek kırık taramasına izin verir.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Köprüleşen segmentte kesinti — anterolateral kemik köprüsü veya sindesmofit boyunca keskin kırık aralığı.
  • Transdiskal hat — füzyonlu disk aralığını kesen, vertebra son plaklarına ve posterior diskoligamentöz yapılara uzanabilen çizgi.
  • Transvertebral hat — korpus trabeküllerini ve korteksini geçen, kimi zaman iki son plağı da ayıran kırık.
  • Posterior eleman devamlılığı — kırık hattının pedikül, lamina, faset veya spinöz çıkıntıya kadar uzanıp uzanmadığını izleyin.
  • Faset ayrışması ve translasyon — posterior eklem uyumunda açılma, subluksasyon veya vertebral dizilim kaybı.
  • Spinal kanal fragmanı — korpus ya da arkus fragmanı kanal içine yer değiştirerek daralma oluşturabilir.

Ankilozan omurga hastalığı AO Spine torakolomber sisteminde M2 durum belirteciyle belirtilir; kırığın morfolojisi yine ilgili A, B veya C yaralanma tipiyle ayrıca tanımlanır.

Normalde

Normal hareketli omurgada disk aralıkları açık ve segmentler arasında hareketli eklemler seçilir; ankilozan segmentte kemik köprüleri süreklidir. Kırık şüphesinde köprü ya da füzyonlu disk içindeki çizgiyi, normal kortikal konturun kesintisini ve aynı seviyedeki posterior elemanların hizasını karşılaştırın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Akut vertebral kompresyon kırığı
Ankilozan omurga hastalarında izole basit kompresyon tipi (AO Spine A tipi) yaralanma olasılığı düşüktür ve bunu instabil B veya C tipi yaralanmalardan ayırt etmek zor olabilir.
Andersson lezyonu, ankilozan spondilitin nadir discovertebral komplikasyonlarından biridir ve yeni mekanik ağrı inflamatuvar hastalık aktivitesine bağlandığında gözden kaçabilir.

Tuzaklar

  • Düşük deplasman kırığın önemsiz olduğu anlamına gelmez; füzyonlu omurga boyunca uzun kaldıraç etkisi kırılgan bir uzun kemik yaralanması gibi davranabilir.
  • Yalnız semptomatik bölgeyi taramak eş zamanlı başka seviyedeki kırığı kaçırabilir; kaynaklar ankilozan omurgada tüm omurga BT veya MR taramasını önerir.
  • DISH'te anterior köprüleşen osteofit de kırılabilir; bu nedenle köprü boyunca kırık hattı aranmalıdır.
  • BT nörolojik bulguya yol açan epidural hematomu veya bağ/omurilik hasarını dışlamaz; nörolojik bulgu ya da açıklanamayan kanal şüphesinde MR eklenir.
