Özet

Yumuşak dokuya penetre olan cam, metal, taş, ahşap veya plastik parçası ağrı, inflamasyon, enfeksiyon ya da nörovasküler yaralanmaya yol açabilir. BT, radyografi veya ultrasonla saptanamayan, özellikle derinde yerleşmiş yabancı cisimleri bulmada ve konumlandırmada yardımcıdır. Metal, cam ve taş çoğunlukla belirgin yoğunlukta görünürken ahşap ve bazı plastikler çevre dokuya yakın atenüasyonda kalabilir; giriş yolu ve çevresel reaksiyon bu nedenle önem taşır. Gözden kaçan yabancı cisimler yara enfeksiyonuna ve kronik yara iyileşme sorunlarına yol açabilir.

Faz ve pencere

Kontrastsız tanısal
Radyografi veya ultrasonla saptanmayan, özellikle derin yabancı cisimlerde BT yararlı olabilir; ahşap ve plastik gibi bazı radyolüsent cisimleri de gösterebilir.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Kemik (G 1800 / M 400).

BT bulguları

  • BT'de yabancı cisimler, özellikle derin yerleşimli olanlar, saptanabilir.
  • Metalik, cam veya taş materyalde çevre yumuşak dokudan belirgin yüksek atenüasyon; ahşap ve bazı plastiklerde değişken atenüasyon.
  • Yabancı cisimle ilişkili enfeksiyonda cilt yüzeyinde görünür veya görünmeyen bir açıklık bulunabilir; eşlik eden selülitte BT'de cilt kalınlaşması ve subkutan yağda kirlenme veya ödem, nekrotizan enfeksiyonda ise gaz görülebilir.
  • Çevresel ödem, yağ dokusunda kirlenme ve reaktif yumuşak doku kalınlaşması; kronik olguda nodüler granülasyon dokusu.
  • Cisim çevresinde sınırlı sıvı koleksiyonu veya gaz; eşlik eden enfeksiyon/apse lehine değişiklikler.
  • BT, penetran yaralanmalarda yabancı cisim parçalarının nörovasküler yapılarla ilişkisini göstermede kullanılabilir.
  • Penetran yaralanma sonrası yabancı cisim birden fazla fragman halinde bulunabilir.

Normalde

Alt ekstremite venlerinde nadir görülen lineer kalsifiye trombüs, BT'de yabancı cisim olarak yanlış yorumlanabilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

  • Ahşap ve plastik her zaman hiperdens görünmez; kemik penceresiyle sınırlı kalmadan yumuşak doku penceresinde de odak ve çevre reaksiyonunu arayın.
  • Metal yabancı cisimler BT ve MR'da artefakt oluşturabilir; cismin tam sınırlarını ve çevre yapıları değerlendirmek için ince kesit ve çok düzlemli görüntüler faydalıdır.
