Coronary motion and step artifact

Eğitim amaçlıdır; hasta başında tanı ve tedavi kararının yerine geçmez.

Özet

Koroner BT anjiyografide hareket artefaktları damar bulanıklığına ve yalancı plak görünümüne yol açabilir; çok kalp atımlı ardışık edinimlerde hacim geçişlerinde basamak artefaktı oluşabilir. Hareket artefaktı stenozu taklit edebileceğinden, koroner lümen ve olası darlık değerlendirmesini güçleştirebilir. Şüpheli koroner plağı farklı rekonstrüksiyon zaman noktalarında ve alternatif görüntüleme düzlemlerinde kontrol ederek hareket artefaktı olasılığını değerlendirin. Hareket artefaktının gerçek lezyon sanılması, önlenebilir invaziv incelemelere yol açabilir.

Okuma sırası

  1. RCA orta segmenti. RCA'yı transversal kesitlerde izleyiniz ve orta segment boyunca konturun netliğini ya da artefakt nedeniyle çiftleşip çiftleşmediğini inceleyiniz.
  2. RCA duvar konturu. Bulanık kenar veya çift lümenin damar anatomisini takip edip etmediğini; özellikle damar hareket doğrultusunda gölgelenme yapıp yapmadığını görün.
  3. RCA eğrisel reformasyonu. Eğrisel düzlemde komşu hacimler arasındaki merkez çizgisi sıçramasını arayın; z-ekseni hizasızlığı basamak artefaktını destekler.

  • Hareket artefaktı koroner damar kenarlarında bulanıklık ve kesitler arası hizasızlık yaratır, bu durum lümen değerlendirmesini zorlaştırır.

Normalde

Koroner stenozu yanlış pozitif değerlendirmemek için bulguyu ikinci bir görüntüleme düzleminde doğrulayın. Aynı segmenti farklı rekonstrüksiyon zaman noktalarında karşılaştırın; plak yalnızca bir zaman noktasında görülüyorsa hareket artefaktı olasılığını değerlendirin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
