Özet

BIA-ALCL, çoğunlukla implant kapsülü çevresinde ortaya çıkan ve özellikle geç başlayan meme şişliği, peri-implant sıvı veya kapsüle komşu kitleyle gündeme gelen nadir bir T hücreli lenfomadır. BT’de tek taraflı sıvı, kapsüler yumuşak doku kitlesi, pektoral/göğüs duvarı uzanımı ve bölgesel nodlar görülebilir; ancak BT ilk değerlendirme yöntemi değildir ve tek başına lenfoma tanısı koydurmaz. Şüpheli peri-implant bulguda ultrasonografi ve sitolojik inceleme tanısal çalışmanın ilk önerilen adımlarıdır. Şüpheli sıvı ya da kapsül kitlesi görüntülemede tanımlanır; kesin tanı sitoloji/histoloji ve uygun immünfenotipleme ile konur. Bulgunun sıradan seroma diye geçiştirilmesi tanısal değerlendirmeyi geciktirebilir, aşırı yorum ise benign sıvıyı kanser olarak etiketleyebilir.

Faz ve pencere

Kontrastsız
İmplant çevresinde asimetrik sıvı koleksiyon, kapsüle yapışık yumuşak doku ve bölgesel lenf nodu büyümesi görülebilir; ancak kesin tanı için patolojik inceleme ve uygun sitolojik değerlendirmenin yapılması esastır.

Önerilen pencereler: Yumuşak doku (G 400 / M 40), Mediasten (G 350 / M 50).

BT bulguları

  • Tek taraflı peri-implant efüzyon — implantla fibröz kapsül arasında hilal biçimli ya da protezi kısmen çevreleyen sıvı koleksiyonu görülebilir.
  • Kapsül kaynaklı solid kitle — implant yüzeyine oturan nodüler yumuşak doku veya kapsül boyunca plak benzeri kalınlaşma saptanabilir.
  • Düzensiz kapsül kalınlaşması — tekdüze ince kapsül yerine fokal asimetri ve nodüler iç yüzey dikkati çeker.
  • Pektoral plan tutulumu — kitle göğüs duvarı fasiyası veya pektoralis kasıyla yağ planını silerek lokal yayılım gösterebilir.
  • Aksiller ya da supraklaviküler nod — bölgesel lenf nodlarında büyüme veya şekil bozulması evreleme açısından önem taşır.
  • Bölgesel lenf nodlarında büyüme veya şekil bozulması izlenebilir; uzak tutulum nadir olmakla birlikte ileri evrelerde değerlendirilmelidir.
  • Sıvının yokluğu lenfomayı dışlamaz — kitle oluşturan alt tipte belirgin efüzyon olmadan kapsüler kitle ön planda olabilir.

Normalde

Normal implantın çevresindeki kapsül ince ve düzgün konturludur; belirgin sıvı veya kapsüle yapışık yumuşak doku kitlesi izlenmez. Karşı implant yatağını kıyas noktası alın; tek taraflı efüzyonun miktar ve dağılımını, kapsül iç yüzündeki nodüler dokuyu ve pektoral fasya ile nodal zincirlerin korunup korunmadığını ayrı değerlendirin.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Benign geç seroma
yalnız sıvı olarak izlenebilir ve BIA-ALCL ile benzer klinik/görüntüleme bulguları gösterebilir; kesin ayırım için sıvının sitopatolojik incelenmesi gereklidir.
İmplant rüptürü
kabuk çökmesi, jel içi serbest çizgiler veya kapsül dışı silikon bulunabilir; efüzyon tek başına rüptür kanıtı değildir.
İmplant enfeksiyonu
çevresel yağ dokusunda inflamatuvar dansite artışı, cilt kalınlaşması veya klinik enfeksiyon bulguları eşlik edebilir.
Meme karsinomu veya implant çevresi karsinom
meme parankiminden kaynaklanan kitle, cilt/pektoral invazyon veya metastatik nodlar yapabilir; kapsül merkezli görünüm ve biyopsi ayrımda önemlidir.
Postoperatif seroma
cerrahiyle zamansal ilişkili, çoğu kez düzgün sınırlı sıvıdır; yeni başlayan veya büyüyen koleksiyonda neoplastik neden dışlanmadan benign kabul edilmez.

Tuzaklar

  • Peri-implant sıvı ve yumuşak doku kitlesi benzer klinik ve radyolojik bulgular oluşturabilir; şüpheli olgularda kesin tanıda ultrasonografi ve sitoloji/histoloji şarttır.
  • BT’de koleksiyon görülmemesi, klinik olarak şişmiş memede BIA-ALCL’yi dışlamaz; olası implant ilişkili lenfomada ilk görüntüleme ultrasonografidir.
  • Peri-implant sıvının tek başına bulunması lenfoma tanısı değildir; kapsül çevresindeki sıvı örneklemesinin sitoloji ve immünfenotipleme ile değerlendirilmesi gerekir.
  • Silikon lenfadenopatisi, implant komplikasyonu olan hastada metastatik nodları taklit edebilir; nod içeriğini ve silikon yayılımı işaretlerini gözden geçirin.
