Özet

Meme kanseri kemiğe sık metastaz yapar; vertebra, kaburga, sternum ve aksiyel iskeletteki odaklar patolojik kırık veya spinal kanal etkilenmesiyle klinik açıdan önemli hale gelir. BT kortikal ve trabeküler kemik yıkımını, sklerotik yanıtı, kırığı ve eşlik eden yumuşak doku bileşenini gösterir; yalnız kemik iliğine sınırlı erken metastazı dışlayamaz. Meme metastazları litik olabilse de mikst ve sklerotik görünüm de mümkündür; tedavi sonrası artan skleroz iyileşme yanıtını yansıtabilir. Kırık veya vertebral arka duvar retropulsiyonu gözden kaçarsa mekanik instabilite ve nörolojik risk eksik bildirilebilir.

Faz ve pencere

Kontrastsız tanısal
Kemik algoritmalı kontrastsız BT’de medüller litik odak, düzensiz skleroz veya mikst litik-sklerotik lezyon; kortikal incelme/kesinti ve patolojik kırık aranır. BT kemik yıkımını ve sklerozu gösterebilir; kemik iliğine sınırlı hastalık gözden kaçabilir. BT kemik iliğine sınırlı erken metastazı tek başına dışlamaz.

Önerilen pencereler: Kemik (G 1800 / M 400), Yumuşak doku (G 400 / M 40).

BT bulguları

  • Osteolitik metastazlar BT'de trabeküler kemikte düşük atenüasyonlu (lisen) alanlar olarak belirir ve artmış osteoklastik kemik rezorsiyonuna bağlı destrüksiyonu yansıtır.
  • Osteoblastik skleroz — medüller alanda yeni veya yamalı yoğun kemik odağı görülebilir; lobüler tümörlerde ve tedavi sonrası değişimde de ortaya çıkabilir.
  • Mikst lezyon — aynı odakta litik destrüksiyon ile sklerotik kemik yapımı bir arada bulunabilir.
  • BT'de görülen litik alanlarda kortikal kemiğin incelmesi veya kesilmesi (yıkımı), metastatik destrüksiyonu ve patolojik kırık riskini düşündürür.
  • Patolojik kırık — metastatik kemikte kırık hattı, kortikal basamaklanma veya vertebra korpusunda çökme gelişebilir.
  • Yumuşak doku uzanımı — kaburga ya da vertebra korteksini aşan yumuşak doku komponenti doğrudan ekstraosseöz yayılımı düşündürür.
  • Omurga arka elemanları ve kanal — pedikül/lamina tutulumu, posterior duvar bozulması veya retropulsiyon eşlik edebilir; BT'de epidural alan sınırlı değerlendirilebilir.
  • Tedavi sonrası skleroz — önceden litik odakta artan yoğunluk kemik iyileşmesine eşlik edebilir; bunu tek başına ilerleme saymayın.

Normalde

Normal vertebra korpusu ve kaburga korteksi kesintisizdir; kemik iliği/trabeküler yapı homojen, korpus yüksekliği korunmuş ve yumuşak doku konturu olağandır. Aynı anatomik seviyedeki komşu vertebralar ve karşı kaburgalarla kıyaslayarak fokal litik ya da sklerotik değişikliği, kortikal süreksizliği ve yükseklik kaybını arayın; tedavi gören hastada önceki BT görünümünü de referans alın.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Dejeneratif değişiklikler
intervertebral disk, son plak ve faset eklemdeki asimetrik dejenerasyon ile son plak sklerozu ve eklem yüzeylerinde bozulmayla seyreder; yapısal değişiklikler arasında erozyon, skleroz ve kemik köprüleşme bulunur.
Enfeksiyöz spondilodiskit
karşılıklı son plak destrüksiyonu ve disk aralığı tutulumu baskın olabilir; metastazda disk genellikle korunur.

Tuzaklar

  • Sklerotik odağı otomatik olarak aktif metastaz saymayın; tedavi sonrası osteoblastik onarım aynı görünümü oluşturabilir ve zaman içindeki değişim önemlidir.
  • Normal BT kemik iliğinde erken metastaz olmadığını kanıtlamaz; klinik şüphe sürüyorsa BT'nin duyarlılık sınırını raporda koruyun.
