Özet

Korpus kallozum agenezisi tam veya kısmi gelişimsel yokluktur; BT'de yapı doğrudan her zaman seçilemese de ventrikül ve orta hat biçimi tanısal ipuçları verir. Kontrastsız beyin BT'sinde paralel ve laterale ayrık lateral ventriküller, oksipital boynuzlarda orantısız genişleme ve yukarı yer değiştirmiş üçüncü ventrikül aranır. Kallozum agenezisi belirti vermeyebilir veya başka nedenle yapılan görüntülemede tesadüfen saptanabilir; eşlik eden beyin malformasyonları klinik önemini etkileyebilir. Kolposefali, oksipital boynuzların frontal boynuzlara göre orantısız genişlemesidir ve hidrosefali ile ayırıcı değerlendirme gerektirir; eşlik eden anomaliler gelişimsel örüntünün parçası olarak ayrıca gözden geçirilmelidir.

Kontrastsız tanısal
Aksiyel ve koronal BT'de corpus callosumun beklenen orta hat konturunun yokluğu/az gelişimi, birbirine paralel ve ayrık lateral ventriküller, belirgin oksipital boynuzlar ve yukarı konumlu üçüncü ventrikül değerlendirilir. Kontrastsız BT ventrikül morfolojisini gösterebilir; kallozumun doğrudan ve segmental değerlendirmesinde MR tercih edilir.

BT bulguları

  • Korpus kallozum agenezisinde kallozal lifler yoktur; doğrudan yapısal değerlendirme için MR, BT'de görülen ventriküler bulgulara ek bilgi sağlayabilir.
  • Paralel lateral ventriküller — normalde öne doğru yaklaşan ventriküler gövdeler orta hatta yakınsamaz ve laterale ayrık seyreder.
  • Kolposefali — atrium ve oksipital boynuzlar frontal boynuzlara göre belirgin geniştir; aksiyel görünüm gözyaşı biçimini alabilir.
  • Üçüncü ventrikül yukarı yer değiştirmiş veya genişlemiş görülebilir.
  • Koronal görüntülerde frontal boynuzlar moose-head görünümü oluşturabilir.
  • Medial hemisferik sulkusların radyal örüntüsü sagittal MR'da sunray işareti olarak görülebilir.
  • Eşlik eden gelişimsel anomaliler açısından tüm beyin gözden geçirilir; kallozum agenezisine posterior fossa ve sulkal anomaliler eşlik edebilir, bazı olgularda polimikrogiri veya subependimal heterotopi görülebilir.
  • Probst demetleri, korpus kallozum agenezisine eşlik edebilen komissür anomalilerindendir.

Ölçütler ve sınıflamalar

Anatomik yaygınlık
Tam agenezide korpus kallozumun tüm segmentleri yoktur; parsiyel agenezide bir veya daha fazla segment eksiktir. Hipoplazi, yapının ince veya az gelişmiş olmasını; disgenezis ise anormal gelişim biçimini tanımlar. MR, kallozal anomaliyi ve eşlik eden yapısal beyin bulgularını karakterize etmeye yardımcı olur.

Normalde

Aksiyel düzlemde lateral ventriküllerin gövdeleri ve atrium/oksipital boynuzların biçimi karşılaştırılır; kolposefaliye özgü orantısız posterior genişleme normal anatomide beklenmez. Koronal/sagittal düzlemde corpus callosum hemisferler arasında rostrum, genu, gövde ve spleniumu içeren kavisli bir köprü oluşturur; şüpheli olguda bu kontur MR ile doğrulanır.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Ayırıcı tanı

Kolposefali hidrosefaliyle karışabilir; oksipital boynuzların genişlemesi morfolojik bir ipucudur ve ventrikülomegali tek başına BOS akımında tıkanıklık olduğunu göstermez.
İzole kolposefali
ventrikül biçimi benzer olabilir, ancak korpus kallozum ve diğer eşlik eden agenezis işaretleri ayrıca araştırılmalıdır.
Septum pellucidum agenezisi
orta hat septal yapılar yoktur, fakat korpus kallozum korunabilir; yapıların ayrı ayrı izlenmesi gerekir.
İnterhemisferik kistlerin bazı alt tipleri üçüncü ventrikülden BOS çıkışını engelleyerek obstrüktif hidrosefaliyle ilişkili olabilir; kistin ventrikülle ilişkisi görüntülemede değerlendirilmelidir.
