Özet

PRES çoğunlukla vazojenik ödemle seyretse de parankimal kanama, sulkal subaraknoid kan ve geri dönüşsüz doku hasarıyla komplike olabilir. Kontrastsız BT akut makrokanamayı, ödem alanındaki hiperdens kanı, kitle etkisini ve infarkt benzeri kalıcı hipoattenüasyonu göstermede ilk basamaktır; mikrokanama ve difüzyon kısıtlılığı MR ile daha iyi değerlendirilir. Posterior ödem içindeki kanama, izole hipertansif hematomdan farklı olarak çevresindeki PRES dağılımı ve eşlik eden sulkal değişikliklerle birlikte okunmalıdır. Hemorajiyi veya doku hasarını atlamak nörolojik kötüleşmenin nedenini eksik tanımlamaya yol açar.

Okuma sırası

  1. Posterior beyaz cevher. Parieto-oksipital beyaz cevherde PRES ile uyumlu düşük atenüasyonlu alanı bulun; bunun karşı lobda ve frontal alanlarda benzer ödemle birlikte olup olmadığını kontrol edin.
  2. Ödem içindeki kan odağı. Ödemli parankim içinde normal gri cevherden daha yoğun odakları ayırın; kanın ödemle aynı alanda yerleşmesi komplikasyon ilişkisini gösterir.
  3. Posterior konveksite sulkusları. Posterior konveksite sulkuslarını ince dilimlerde takip edin; çizgisel hiperdensite varsa kanın parankim dışındaki sulkal dağılımını ayrıca işaretleyin.
  4. Ventriküller, orta hat ve posterior fossa. Son olarak lateral ventrikülleri, orta hattı ve posterior fossayı kontrol edin; ventrikül basısı, itilme veya hidrosefali varsa eşlik eden kitle etkisini belirtin.

Faz ve pencere

Kontrastsız tanısal
PRES ile uyumlu hipoattenüe ödem alanının içinde veya komşuluğunda hiperdens intraparenkimal kan, sulkal subaraknoid kan ya da kanla karışık ödem görülebilir. Kontrastsız BT ödem alanlarını gösterebilir ancak sitotoksik ödemi vazojenik ödemden ayırt etmekte yetersizdir; kesin tanı ve hasar karakterizasyonu için manyetik rezonans görüntüleme gereklidir.

Önerilen pencereler: Beyin (G 80 / M 40), Subdural (G 200 / M 75).

BT bulguları

  • Ödem içi intraparenkimal kan — PRES'te parieto-oksipital bölgede subkortikal kanama görülebilir; konveksite subaraknoid kanama eşlik edebilir.
  • Sulkal subaraknoid kan — posterior konveksite sulkuslarında çizgisel hiperdensite şeklinde ortaya çıkabilir.
  • Peteşiyal kanama — ödemli parankim içinde küçük dağınık hiperdens odaklar makrohematomdan daha belirsiz olabilir.
  • İnfarkt benzeri kalıcı hipoattenüasyon — PRES infarktla komplike olabilir; BT'de hipoattenüasyon görülebilse de vazojenik ve sitotoksik değişiklikleri güvenilir biçimde ayırt edemez.
  • Kitle etkisi — ödem ve hematom birlikte giral şişme, ventrikül basısı ve orta hat itilmesi oluşturabilir.
  • Posterior fossa kanaması/ödemi — serebellar şişme dördüncü ventrikülü daraltabilir; küçük arka çukur hacmi nedeniyle bası etkisi önemlidir.
  • Eşlik eden PRES dağılımı — kan odağı dışında iki taraflı posterior veya santral vazojenik hipoattenüasyon komplikasyonu bağlama oturtur.

Ölçütler ve sınıflamalar

İntraparenkimal hemoraji
Kan odağı ödemli beyin parankimi içinde yer alır; küçük peteşiyal odaklardan kitle etkisi yapan hematoma kadar değişebilir.
Sulkal subaraknoid hemoraji
Kan konveksite sulkuslarını çizgisel olarak doldurur; parankim içi hematomdan anatomik olarak ayrılır.
İnfarkt benzeri doku hasarı
Vazojenik ödem alanında sitotoksik bileşen gelişmesiyle difüzyon kısıtlılığı ve kalıcı doku hasarı ortaya çıkabilir; BT tek başına erken ayrımı güvenle yapamaz.

Normalde

Komplikasyon şüphesinde hiperdens odağın çevresindeki düşük yoğunluklu ödemi, karşı lobu ve ventrikül/orta hat konumunu kıyaslayın; odak çevre ödeminden bağımsız, derin tipik hematom yerleşimindeyse alternatif nedeni düşünün.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.

Anevrizmal subaraknoid kanama
bazal sisternalarda ve fissürlerde yoğun kan dağılımı konveksite ağırlıklı PRES ilişkili sulkal kandan ayrılır.
Travmatik kontüzyon
frontal veya temporal yüzeylerde, travma mekanizmasına uygun kortikal-subkortikal kanama ve eşlik eden kırık/ekstraaksiyel kan aranır.

Tuzaklar

  • PRES kanaması yalnız büyük hematom olarak görülmez; ödemli posterior sulkuslarda ince subaraknoid kanı ve parankim içindeki küçük hiperdens odakları tarayın.
  • Hemorajik PRES'i primer hipertansif kanama diye sınıflandırmadan önce çevredeki iki taraflı ödemi ve dağılımı kontrol edin.
  • PRES'te küçük odaksal kanamalar görülebilir; kanama bulgularını değerlendirirken yalnız büyük hematomları aramakla yetinmeyin.
  • Düşük atenüasyonun vazojenik ödem mi sitotoksik değişiklik mi olduğunu BT ile güvenilir biçimde ayırt etmek mümkün değildir; infarkt benzeri hasar şüphesinde MR ile değerlendirme gerekir.
