Tonsillar herniation

Özet

Arka çukurda yer kaplayan lezyon, yaygın beyin ödemi veya BOS dolaşım bozukluğu bulunan hastada serebellar tonsillerin foramen magnuma doğru itilmesi alt beyin sapını ve üst servikal medullayı sıkıştırabilir. Kontrastsız beyin BT'si akut kitle etkisini, tonsil düzeyindeki kalabalıklaşmayı ve eşlik eden hidrosefaliyi hızla gösterir; tonsillerin konumunu ve nedenini karakterize etmekte MR daha duyarlıdır. Bu görünüm akut kötüleşmede kritik bir bası işareti olarak raporlanmalıdır. Daralma ya da herniasyon gözden kaçarsa solunum-dolaşım merkezleri ve beyin sapı üzerindeki yaşamı tehdit eden bası tanınmayabilir.

Okuma sırası

  1. Foramen magnum. Sagittal orta hat görüntüsünde foramen magnumu belirleyin ve serebellar tonsillerin bu düzeye göre konumunu izleyin.
  2. Serebellar tonsiller. Midsagittal görüntüde McRae hattını referans alarak tonsil ucunun bu hattın altında olup olmadığını ve iniş miktarını değerlendirin.
  3. Sisterna magna ve alt medulla. Foramen magnum çevresindeki BOS aralıklarının daralıp daralmadığını ve sisterna magnanın silinip silinmediğini değerlendirin.
  4. Dördüncü ventrikül ve posterior fossa. Dördüncü ventriküldeki basıyı ve eşlik eden serebellar şişliği değerlendirin.

Kontrastsız tanısal

İyotlu kontrast bu herniasyon paterninin gösterilmesi için gerekli değildir.
Kemik algoritması
Oksipital kemik ve foramen magnum kenarları, bazion-opistion hattı çevresindeki kemik anatomi ve kraniyoservikal travmatik kırıklar yeniden yapılandırılır; parankim yumuşak doku ayrıntısı bu algoritmada sınırlıdır.

BT bulguları

  • Foramen magnuma doğru tonsil uzanımı — tek veya iki tonsilin normal tonsiller fossadan kaudale sarkması ve çıkış düzeyinde kalabalıklaşma.
  • Sisterna magna daralması — tonsiller ile oksiput/üst servikal kanal arasındaki hipodens BOS mesafesinin incelmesi ya da seçilememesi.
  • Alt medulla basısı — medulla konturunda düzleşme veya kraniyoservikal bileşkede öne-arkaya sıkışma.
  • Arka çukur kitle etkisi — serebellar ödem, hematom, tümör veya kitle çevresindeki dördüncü ventrikül deformasyonu.
  • Obstrüktif hidrosefali — lateral ve üçüncü ventriküllerde genişleme ile dördüncü ventrikül çıkış düzeyinde akım engeli.
  • Eşlik eden beyin ödemi — gri-beyaz madde ayrımının azalması ve arka çukur folialarının silinmesi.
  • Kraniyoservikal kemik yaralanması — oksipital kondil ya da foramen magnum komşuluğunda kortikal basamaklanma ve kırık hattı.
  • Akut herniasyon kuşkusunda tonsillerin kaudal yer değiştirmesini, BOS aralıklarının daralması veya silinmesi ve beyin sapı ya da arka çukur bası bulgularıyla birlikte değerlendir; tonsil iniş miktarı tek başına yeterli değildir.

Normalde

Normal kraniyoservikal bileşkede tonsiller foramen magnumun üstündedir; foramen magnumdaki BOS alanının daralması ise posterior fossa kalabalıklaşmasıyla ilişkili bir bulgudur. Chiari I için sık kullanılan radyolojik ölçüt, midsagittal MR'da tonsillerin foramen magnumun ≥5 mm altına uzanmasıdır; bu eşik tek başına klinik anlamı belirlemez ve akut herniasyon kuşkusu BOS aralıkları ile beyin sapı ve arka çukur bası bulgularıyla birlikte değerlendirilir.

Normal BT ile kıyasla

Aynı bölgenin, kaynak veri setinde patoloji içermediği belirtilen bir BT incelemesini kesit kesit kaydırın; organ sınırlarını açarak anatomiyi hasta görüntüsüyle karşılaştırın. Küçük rastlantısal bulgular tümüyle dışlanmamıştır.
